APPENDIX A NOTICE OF VIOLATION Georgia Power Company License No. CPPR-108 Plant Vogtle Based on the NRC inspection May 15-16, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50 and implemented by VNP PSAR Section 17A.5, verification that work is accomplished in accor-dance with approved instructions, procedures and drawings is obtained through various levels of surveillance, inspection and audits.
Georgia Power Company Vogtle Field procedure GD-T.01 noncoaformance control requires that a nonconformance when identified shall be reported as directed by the procedure.
Contrary to the above, on May 16, 1979, during the placement of 1200 cy Pour A-08D-003C, two batches of concrete were rejected at the batch plant by the QC inspector. The two batches in question were 8 lbs. and 17 lbs. over the maximum tolerance for aggregate.
The placement QC inspector was instructed by the engineering staff to place these two batches. No nonconformance report or other documentation was prepared to justify the placement of these two batches of concrete.
This is an infraction.
