APPEh3IX A NOTICE OF VIOLATION South Carolina Electric and Gas Company License No. CPPR-94 V. C. Summe-Nuclear Station, Unit 1 Based on the NRC inspection May 22-25, 1979, certa _ a of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in c'>rrespondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50 and implemented by South Carolina Electric and Gas Company (SCE&G) FSAR Section 17.1.5, appropriate instructions will be used for activities af fecting quality.
SCE&G Field Quality Control Procedure (FQCP) 7.1.7 requires inspection
'of drilled-in expansion bolts for safety-related cable tray supports to be in accordance with FQCP 7.1.5.
FQCP 7.1.5 requires an expansion bolt installation checklist, Form 7.1.5-1, to be completed by QC inspection personnel during the expansion bolt installation.
Contrary to the above, all required sections of the expansion bolt installation checklist, Form 7.1.5-1, had not been completed for the following safety-related cable tray supports:
1.
Support number 304 for cable tray number 2058.
2.
Support number 42-15 for cable tray number 2032.
The expansion bolt checklists had been signed by the QC inspector and had been reviewed and approved by the QC supervisor.
This is a deficiency.
