Appendix A NOTICE OF VIOLATION The Cleveland Electric Docket No. 50-440 Illuminating Company Docket No. 50-441 Based on the inspection conducted on May 22-24, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. This item is an infraction.
10 CFR 50, Appendiv B, Criterion X, requires that, a program for inspection of activities affecting quality shall be established and executed by or for the organization performing the activity to verify conformance with the doctmented instructions, procedures, and drawings for accomplishing the activity.
The PNPP PSAR Chapter 17, Section 17.1.10 states in part that,
" Quality will be controlled by inspection or process control or a combination of both as necessary Contrary to the above, the NRC inspector observed two separate instances in which licensee personnel failed to execute the program for inspection of activities affecting quality to verify conformance with documented instructions and procedures:

Immediately prior to concrete placement, the following specific conditions adverse to quality had not been corrected in con-crete placement No. AX2-W05A-650.
The pour area had been inspected and released for concrete placement by responsible QA/QC personnel in accordance with NECC Procedure QP 10.1, Rev.

a.
Contamirated reinforcing steel.
b.
General unacceptable cleanliness of the horizontal construction joint.
c.
Embedded duct tape in the vertical keyed construction joint.
2.
A total of five separate concrete placements over a period of two days were not being cured as required by Specification SP-201 and ACI 301-72.
