Attached are three signed originals. d 40 conformed copies of License Change Application 55 requesting amendment of Operating License NPF-1.
This License C'aange Application has been evaluated to be a Class II amendment request and a check for $1200 is attached in accordance with 10 CFR 170.
PGE has transmitted a copy of this request to the Oregon Depactment of Energy for their consultation.
Also attached is one signed copy of a Certificate of Service for License Change Application 55 cc the chief executive of the county in which the facility is located and the Director of the State of Oregon, Department of Energy.
