APPENDIX A NOTICE OF VIOLATION Jersey Central Power and Docket No. 50-219 Light Company Based on the results of an NRC inspection conducted on April 10-12, 1979, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-16, as indicated below.
This item is a deficiency.
Technical Specification 6.8.1 states in part, " Written prs _adures shall be established, implemented, and maintained...."
Procedure No. 108, Revision 10, February 9, 1979, Control of Tagging, Electrical Jumpers, Lifted Electrical Leads, Keys and Locked Valves, Paragraph 5.4.5 states in part, "The individual from mairtenance installing or removing elec-trical leads shall complete all electrical lead data portion of the check-off sheet.
Contrary to the above, as of April 12, 1979, the Lifted Lead index ar.d Check off sheet, number 38/78, initiated on November 20, 1978, indicat d that the Reactor Vessel Wide Range Level Indicator lead, which had been reconnected, was still disconnected.
