t APPENDIX A NOTICF 0F VIOLATION Mississippi Power and Light Company License No. CPPR-118 Grand Gulf 1 Based on the NRC inspection April 3-6, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50 and implemented by Mississipp: Power and Light PSAR Section 17.1.5, "The Mississippi Power and Light Quality Assurance Program requires that activities affecting quality be accomplished in accordance with documented instructions, procedures and drawings". Work Plan / Procedures Manual, Volume 1, WP/P-1 states in part that, " Superintendents and Engineers are responsible for assuring work is accomplished in accordance with the latest design document revisions logged in the Document Control Register...", and WP/P-5 states in part that "the responsible Field Engineer shall initiate a CR/N (Change Request / Notice) to request a change in project engineering design documents...".
Contrary to the above, on April 5,1979, the installation of the Main Steam Moment Guides in the Auxiliary Building Exterior Wall was not being performed in accordance with the latest revisions of the Design Documents (Drawings '-130E, Revisior 2 and C-1310F, Revision 2) listed in the Document Control Register. Discussions with the responsible Superintendent and Field Engineer indicated that verbal approval had been given to the Superintendent to procede with the work pending receipt of a drawing change.
The verbal approval was given in lieu of an approved CR/N.
This is an infraction.
