Appendix A NOTICE OF VIOLATION The Curators of the License No. 24-00513-32 University of Missouri Based on the inspection conducted on June 18-22, 1979, it appears that certain of your activities were in noncocpliance with NRC requirements, as noted below.
Items 1 through 3 are infractions.
1.
Faragraph 19.12, Title 10 of the Code of Federal Regulations, requires that you instruct all individuals working in your restricted areas in the applicable provisions of the Co==ission's regulations and licenses.
Contrary to this requirement, on the day of the inspection, custodians working in your restricted area had not been instructed in the applicable provisions of the regulations and the conditions of your license commensurate with potential radiological health protection proble=s.

Paragraph 20.205(b)(1), Title 10 of the Code of Federal Regulations, requires that upon receipt, specified packages of radioactive
=aterial, including lodine-131 in excess of 100 millicuries, shall be =onitored for radioactive contamination caused by leakage.
Contrary to the above, on October 16, 1978, and May 4, 1979, monitoring was not perfor:ed on two separate packages, each containing 105 and 150 =1111 curies, respectively, of iodine-131 in liquid form.

Condition 26 of your license requires that licensed material be possessed and used in accordance with the statc=ents, represcatations and procedures contained in your application and letter dated December 23, 1972.
One of these requirements, Section 2.2.2 of the Handback of Operaticas (Radiation and Radioactive Waste Pamphlet) states:
"All transfers and diluticas shall be in functioning exhaust hoods or glove boxes, unless it is cc:pletely safe to do otherwise".
Centrary to this requirement, on May 5, 19,<,
a persca opened a 35 millicurie iodine-131 solution on a bench in the Nuc} ear Medicine Depart ent for the purpose of withdrawing and transferring an aliquot of iodine-131 for a patient dose.
This person's thyroid count, perfor ed on May 6,1977, indicated an elevated iodine-131 thyroid uptake, from :ero to 64 MFC-hours within a seven day period.
