SU" MARY OF KEY EVENTS Week Ending July 13, 1979 Bic Rock Point Consumers Power Company (CPC) informed NRC that a U.S. Air Force B-52 made a lo level run directly over the plant.
This run was in violation of a commit-ment made by the U.S. Air Force to CPC, and is the first occurrence since 1971.
CPC has sent a telegram to D0D requesting adherence to the com-itment.
Prairie Island Unit i The NRC staff has been notified that the vibration in the turbine generator that caused shutdown of this unit on July 4 resulted in some demage to turbine blades and casing. The licensee expects to restart the unit July 25, 1979.
Pilcrim The plant was shut down July 11 to repair a leak in a control rod drive return line.
It is expected that repair will be co.pleted and the unit will be re-started by July 12.
R.E. Ginna The plant shut downJuly 6 for inspection of cracks found in the feedwater pipe and steam generator nozzle welds.
New elbows will be installed and the picnt will be shut down until about August 6,1979.
Edwin I. Hatch The plant tripoed off the line July 11 due to generator over excitation and auxiliary differential lockout upon failure (high) of the TG voltage reculator.
Repairs are being made to the regulator with the reactor in hot standby.
Re-sumotion of clant startup is expected by July 13, 1979.
Fish Kill at Salem i At the plant, weakfish were impinged in large numbers on the intake "aveling screens.
The unit is expected to be on line by August 8.
Meanwhile, the staff is kept inforned of the daily ir;ingerent rate by the utility.
Studies of the weakfish copulation in the plant vicinity are unterway to determine the signifi-cance of the intake losses.
Ionizine Radiation Acree-ent The Interagency Co -ittee on Federal Guidance for Occupational Exposure to Ionizing Radiation reached an agreement on guidance to be proposed for ccntrolling occucational excesure to external sources cf radicticr Mencan Assignee Leopoldo Nieto from the Mexican National Nuclear Safety and Safeguards Co riission will becin a 3 z week assignment on July 16 with NRR.
Mr. Nieto is the first of a srall croup of Mexican assicnees who will be working at NRC on the Laguna Verde Plant SAR over the next few months.

.ieam condensing line ha.d be:n found to exceed structural support limits under calculated DSE conditions. These supports are in the process of being todified now during a scheduled maintenance outage for a snubber inspecticn on Unit No. 2.
Similar supports on Unit No.1 were attached dif ferently and trere acceptable. Also, two (2) HPCI 1" drain lines on
nit No.1 and a 1/2" head vent line on Units Nos.1 and 2 were found to exceed 1.8 Sh (FSt.R allo.zable). CP&L is evaluating modifications to upgrade these lines r.ow.
Point Eeach Unit 2 As a result of an inspection conducted on July 2,1979, cracks have been discovered in feec< tater line welds at Point Beach Unit 2.
The cracks exist in btth ends of a 14" x 16" reducer which joins the feedwater piping to the steam generator nozzle.
Repairs are underway. A meeting is scheduled for July 6,1979 with the NRC staff in Eethesda to discuss the details of th-repair program.
Kewaunee As a result of an inspection conducted during the current refueling outage, cracks have been fcund in feedwater line welds at Kewaunee, and are being repaired.
A meeting was held with the NRC staff on July 3,1979 to discuss the cracks. The licensee vas asked to submit a sum.ary report prior to startup that will include further details about the cracks and repairs, and a commitment to inspect the repairs during the next refueling.

The union requested the meeting, which was offered by faC letter of April 13,1979, ';hich responded to their petition to the Com.issicn to take accion on their protest.
Eig Eock Point Consumers Power Cc"pany (CPC) informed us on July 6,1979, that at about 3:30 p.m. on July 5,1979, a U. S. Air Force B-52 made a low level run directly over the Big Rock Point Plant. This was in violation of a commit-rent r. ace by the U. 5. Air Force to CPC on February 19, 1971, and is the
'irst occurrance since 1971.
CPC has sent a telegram to DOD reouesting a narence to the com.itment.
CPC has received an inquiry on this incident from the Office of Senator Levin and press coverage is anticipated.

Three Mile Island 1 - Uccate Pipe cracks have.been found in the Spent Fuel, Decay Heat, Suilding Spray and Make-up systems of the Three ;',ile Island L' nit i fa c il i ty.
The cracks occurred in the v. eld heat effecud :cne of Type 304 stainless steel material, initiating on the insid surface of the pipe and appear to be typical intergranular stress cercosion cracking similar to those fcund in B'.iR primary system piping and other i.?R secondary sys te piping.
The extent of cracking in he.;e systms has not been cetermined since exami-nations have not bcen cc pleted.
't.ever, result-to date indicate that the cracking is confined to stacnant fluid lines containing borated water.
As of July 5, 1979, 651 pipe welds had been exc,.:ir,ed by ultrasonic testing and 137 indications of cracks were found.

due to a grid disturbance in Florida.
Prior to the cycnt Georgia Pc'. er Cor.pany was supplying about 500 Mw to the riorida grid. The Hatch turbine generator tripped due to Generator over excitatior, and Auxiliary Differential lockout upon failure (high) of the TG voltage regulator.
The reactor plant did not trip 2r.

Ralph Jones and Don Hopkins met with representatives of NMSS, I&E and the Department of Transportation to discuss the actions to be taken in response to the edict of the three Governors in whose states there are low level waste burial sites.
It appears that the best approach is to amend NRC regulations to bring those licensees shipping these wastes under the NRC regulations.
They are now exempt from NRC control if they are subject to DOT regulations.
This would permit NRC inspection and enforcement of the DOT standards to satisfy the Governors' concerns.
Further SD action is dependent on NMSS and DOT responses and decisions.

Interagency Committee on Federal Guidance for Occupational Exposure to loni3ing Radiation - Meeting, July 6,1979:
The Interagency Committee on Federal Guidance for Occupational Exposure to Ionizing Radiation met on July 6,1979, in an attempt to reach an agreement on a method for calculating occupational limits on the concentration of radionuclides in air (presently called Maximum Permissible Concentrations or MPC) to be called Derived Air Concentrations or DACS, in the new guidance.
Agreement on this guidance is needed to permit EPA, OSHA, and NRC to move ahead with plans for a joint hearing on the adequacy of present standards for occupational radiation protection. Agreenent was reached last week on guidance to be proposed for controlling occupational exposure to excernal sources of radiation.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending July 13, 1979 US/IAEA Safeauards Acreement At the ratification hearings on June 22, the Senate Foreign Relations Committee staff provided the Executive Branch 3 list of questions and asked that answers be submitted for the record. Also du,ing the course of the hearings, the Executive Branch submitted for the rec m their response to the questions raised in the analysis of the US/IAEA Safeguards Agreement which was performed by the Congressional Research Service (CRS).
Mr. Paul Morruw has been working with other NRC offices and the Executive Branch in drafting responses to the hearing questions and in revising the text of the Executive Branch response to the CRS questions.
U.S. Safeguards Trainino Course Messrs. Partlow and Wirfs attended a DOE /NRC steering group meeting on July 9 at Germantown to review a proposed action plan and course syllabus for developing and instituting a U.S. training course on state systems of accounting and control for foreign countries.
The course is to be offered in response to the requirements of Section 202 of the NNPA whereby DOE, in consul ta tion with NRC, is required to establish and operate a training program in safeguards and physical security.
In addition to representation on the steering group, DOE is now requesting that NRC provide two members to participate in a working group that is to be established to work with LASL in the implementation of this program.
Local Ordinances to Restrict Nuclear Shinnents We are responding to a number of letters from officials of local governments and public interest groups who are considering supporting local ordinances to restrict nuclear shipments through their respective localities.

Xe-133 Release to the Public and Unauthorized Burial of Iodine-125 - Two events were reported: (1) a release to the unrestricted area on April 10, 1979, of 4.6 millicuries of xenon-133 - no member of the public was in the -rea at the time; and (2) the disposal on May 25, 1979, of 5 to 10 millicuries of iodine-125 in a properly labeled waste drum to normal trash.
The Region plans to conduct a special inspection.

A liquid radioactivity release to Lake Keowee occurred on July 7, 1979, when an operator erroneously valved the secondary ride cf the Low Pressure Injection heat exchanger A to the service water radiation monitor line.
Maximum permissible oncentrations were not exceeded.
The principal isotope was cob 31t-58.

PNO-79-225 Browns Ferry Unit 1 - Inadvertent Release of Low Level Activity - During routine surveillance testing on July 6, 1979, a Residual Heat Removal system service water alarn indicated a radioactive release from Unit 1.
The RHF heat exchanger was pronptly isolated and a hydrostatic test of the heat exchanger showed no leaks.
Subsecuent evaluation indicated that the activity was from a primary to service water leak which had occurred about 2 years ago.
The former leak apparently deposited low level activity in the service water side cf the heat exchanger which is periodically released due to changes in heat exchanger operation (Closed) 1.
PMO-79-226 Jamesport Unit 1& 2 - Fatal Accident on Site -
On July 6,
'979, a trop duster helicopter hit a guy wire for the meteorological tower, killing the pilot.
The tower was not seriously damaged.

The plant is under construction and there is no nuclear fuel on-site.
An IE inspector is on-site and will review the licensee's action.

Ginna Unit 1 - Cracks in Steam Generator Feed Water Piping - Ultrasonic and radiographic examination of both
("A" and "B")
steam generator feedwater nozzle-to-pipe welds have revealed cracking in the base material _of the 90. degree elbow which attaches to the feedwater nozzle.
The crack indications are estimated to be 1/32 - 1/16 inch in depth originating from the inside surface of the elbow.
The licensee is in the process of continuing to examine the remainder of the feedwater piping welds inside the reactor containment.
The plant will be out-of-service for at least three weeks.

Daring routine maintenance in the steam tunnel (outside of the primary containment) on July 20, 1979, personnel noted water dripping from piping insulation (lagging) around the 3 inch Control Rod Drive return line.
The lagging was removed and visual examination revealed a through-wall leak in the downstream weld on the isolation valve in the 3" return line.
The leak is not isolable and is reportedly leaking at the rate of 4-5 Pups per hour.
Reacto; shutdown for repair began during the norning of July ll, 1979.

PNO-79-231 Arkansas Nuclear One Unit 1 - ANO Unit 1 Turbine Vibrations - On July 10, 197 9, Unit 1 was shutdown at 6:52 a.m.
from 21% power due to abnormal turbine vibrations.
The initial investigation has not revealed the source of the vibrations.
The licensee estinates that the plant shutdown could be up to three weeks.

PNO-7 9-2 3 2 Three Mile Island Unit 2 - Discovery of Airborne Betc Activity in the Auxiliary and Fuel Handling Building -
On July 4, 1979, the licensee contractor determined that a filter paper from a portable air sampler in the Fuel Handling Building had radiation levels of 1.2 Rad per hour on contact winn a portable radiation survey meter.
NRC inspectors initiated confirratory measurements and verified that high beta activity was present in the Auxiliary and Feel Handling Building.
Records show that approximately 97 individuals entered the subject area during the period of high airborne activity, wearing as a minimum, full protective anti-C clothing and full f ace respirators.
Urine, fecal and whole body counting analyses of selected individuals who entered the area will be conducted.
Further evaluation and follow-up by the licensee and NRC are in progress.

Preliminary medical evaluation indicates a pcssibly fractured thigh.

PNO-7 9-2 34 Vermont Yankee - Erratic Behavior of Recirculation Pu=p - At about 3:15 p.m. on July 11, 1979, erratic speed on Recirculation Pump "A" was noted which caused changes in core flow with resultant changes in the reactor power level of less than percent.
The reactor was operating at 98 percent power.
The problem was found to be variations in the Motor-Generator set generator output voltage caused by the M-G Set tachometer.
The tachometer was replaced and the plant was back to 95 percent power by 8:00 a.m.

C. Cook Unit 1 - Reactor Coolant Pu:np s - Excessive Seal Leakage - On July 5, 1979, while performing low power physics testing, excessive reactor coolant pump seal leakage was experienced on pumps 3 and 4.
Following the cor.pletion of the low powcr physics testing on July 10, the licensee attenpted to resent the seals by varying the differential pressure acrasr tne seal faces (Westinghouse procedure) which was unsuccessful.
Even though the seal leakage did not exceed the Techspec limit,
the licensee has alected to return to cold shutdown to examine and/or replace these seals.
This is expected to require 2 to 3 days fer completion.

PNO-79-240 Wolf Creek Unit 1 - Contamination of Safety Related Stainless Steel Piping - A construction deficiency occurred in which safety related stainless stee} piping was splat red with fire proofing material which contained a chloride conttninant.
The fire proofing material, "Duraspray," is a magnesi2m oxychloride and is sprayed on bare carbon steel structural members as a fire proofing coating.
During the spraying application, the material splattered onto uninstalled saf et; related stainless steel piping. Duraspray actively corrodes, upon contact, bare stain.Less steel, copper and aluminum.
Tie A/E (Eechtel) is evaluating the effect of the material on the piping and is formulating a procedure for removal of the material.

Arrest of Union Members During Operators Strike - At 8:20 a.m.
on July 11, approximately 120 union members were arrested without incident at the plant entrance by State and County officials for not complying with a Restraining Order issued q the State Circuit Court Judge which limited picketing to no closer than 500 yards of the plant entrance. PNS-79-74 Pilgrim Unit 1 - Breach of Security - A radwaste shipment truck arrived at the site at 7:20 a.m.
on July 11 and was searched and checked for contamination and authorized entry into the protected area.
Later the 15 year old nephew of the driver was discovered asleep in the cab of the truck and was taken to the guard house under escort.
The maximum possible dosage to the 15 year old was estimated to be 4 nr.
Disciplinary action was taken against the guard and the truck driver was disqualified by the licensee as a qualified hauler.
NRC inspectors in the vicinity were dispatched to the site to review causes and corrective action taken. (Closed) aa.
PNS-79-75 Nuclear Fuel Servrees, Inc., Erwin, Tennessee -
Arrest of Union Members Duril.g Operators Strike - This Preliminary Notification updates PNS-79-73.
The restraining order has been_ relaxed and it now permits two union members to picket at the plant entrance.
The IE Resident Inspector assisted by three Safeguards Inspectors continue to monitor plant operations and security on all shifts.

OFFICE OF THE EXECUTIVE LEGAL DIRECTOR ITEMS OF INTEREST FOR THE WEEK ENDING JULY 13, 1979 Skagit Nuclear Power Project, Units 1 and 2 On July 9, the Appeal Poard issued a Memorandum and Order ( ALAB-552), in which it detennined that certain Indian tribes had failed to set forth adequate good cause why their petition to intervene was untimely.
The Appeal Board ruled that in the instance of a very late petition, the strength or weakness of the justification for the delay may prove crucial, since the greater the tardiness the greater the likelihood that the addition of a new party will delay the proceeding. The Appeal Board indicated that while the delay factor may not be conclusive in determining whether a petition will be denied, it is an especially weighty one.
However, the Appeal Board exercised discretion to permit the Indian tribes to file a supplemental memorandum to attempt to cure the defects in their argument that they were furnished erroneous informa-tion by the NRC or the Department of the Interior on matters of basic fact and that it was reliance on that information which prompted Petitioners' inaction until the time that they filed their petiticn.
St. Lucie Nuclear Power Plant, Unit 2 On July ll, the Appeal Board issued a Memorandum and Order ( ALAB-553) granting the Staff's motion for an extension of time in answering the Board's inquiry concerning the matter of the stability of the Applicant's electrical grid and the adequacy of the f acilities' emergency power systems generally.
Staff's response was originally due May 21, 1979.
It was extended until May 22, 1979, because of unavailability of technical staff due to the Three Mile Island com-mitment.
AL AB-553 granted additional time to September 21, 1979 for the satae reason.
In addition, the Appeal Board called the matter to the attention of the Commission for any action which the Commission deemed appropriate.
Hartsville Nuclear Power Plant, Units l A, 2A,1B, 2B In a Decision ( ALAB-554) dated July ll, the Appeal Board:
(1) vacated the Licensing Board's grant of summary disposition because such a grant was made af ter the development of the evidentiary record and was inappropriate; (2) granted judgment on the issue of operational effects of the proposed plant on the rare and endangered specie Lampsilis orbiculata because a biological opinion had been obtained from the Depar tment of Interior; and (3) deferred a decision on the issue of the effects of coristruction activities on the Lampsilis orbiculata_ pending presentation of supplemental memoranda by the Applicant and the Staff concerning the acceptable standard for sedimentation levels during construction.

Mr. Nieto is the first of a sraall group of Mexican assignees who will be coming to NRC to work on different sections of the Laguna Verde Plant SAR over the next few months.
Procosed Arranaement Extensions IP is preparing letters to France and Japan proposing 5-year extensions of our bilateral regulatory information exchange arrangements, which had expired on June 23 and May 30 respectively because of now-resolved questions about whether and how to incorporate nonproliferation language into NRC's international agreements.
Similar letters will be sent to Spain, Sweden, and Switzerland (all due to expire by year's end) within the next few weeks.
Foreign Report Received Two sets, 6 volumes each, of the Environmental Report for the Philippine Nuclear Power Plant Unit I have been received.
One set is being made available to the Commissioners and their staff through the Secretary.
The other set will remain in IP for use by other staff offices and for public use until it is placed in the Public Document Room.

2 EXPORT / IMPORT AND INTERNATIONAL SAFEGUARDS Pacific Basin Spent Fuel Storage Study Group On July 13 IP and NMSS representativer attended at State Department a meeting of the " Pacific Basin Spent Fuel Storage" Study Group which was chaired by Marvin Moss (DOE) and Dick Scribner (State).
The meeting was called to clean up some unresolved issues prior to commencement of discussions with Japan regarding potential stcram sites for spent fuel in the Pacific.
(The Commission may recall that the South Pacific Forum meeting held in Honiara July 9-11 condemned any move to use the Pacific as a dumping group for nuclear waste.)
NRC was requested to lend assistance in reviewing the recently issued draft study report for safeguards logic, licensing considerations and other pertinent regulatory issues.
Other agencies present included State, ACDA, USGS, DOE, and Interior.
1980 and 1981 Fundina for RERTR (Reduction of Enrichment of Research and Test Reactors)
On July 14 IP attended a meeting at DOE, chaired by John Deutch, on the subject of 1980 and 1981 RERTR funding.
The principal issue related to DOE's request to keep the project funded at the present $5 million/ year level, excluding the $1 million/ year SSOD funds that are currently lumped by OMB into DOE's
$5 M budget.
Since the spending of SS0D dollars is constrained by law to specific tasks (e.g., foreign demonstration projects), thereby depriving DOE of its program management flexibility, DOE prefers to have the SS0D funds trans-ferred to State's 10 budget (International Organizations).
In this they were supported by Lou Nosenzo, who promized to push for this transfer.
Presently projected reactor conversion schedulet are not expected to be affected by the above switch of funds.

Cn Friday evening, June 29, Thomas C. Elsasser accompanied Commissioner Gilinsky to a public meeting conducted by Congressman Allen Ertel in Middletown, Pa. to assist in answering public inquiries regarding the Three Mile Island site.
On Friday, July 6, Andy Robart, Region V SLO attended the second in a five part series of hearings in Phoenix conducted by the Arizona House of Representatives Special Interim Committee on Atomic Energy.
The agenda for this particular hearing was taken up by various u tility witnesses explaining the need for and role of nuclear energy in Arizona.
Other witnesses included Dr. Harold Lewis; Commissioner Ron Doctor, California Energy Commission; David Mastbaum, Environmental Defense Fund and Bruce Meyerson, Arizona Center for Law and the Public Interest.
Two general legislative concerns were frequently voiced; water needs to generate electricity and power exports to California.
The NRC has been asked to provide assistance on future hearings to be held in September on the subjects of Accident Prevention, Accident Response and the relationship of Three Mile Island to the Palo Verde Nuclear Power Plant.

Further, the individual program offices should also have an operational data analysis and evaluation capability.
Such a capability would allow the individual program office to make input to the agency-wide office, review and comment on the agency-wide office's evaluations, and perform special operational safety data analysis.
