50-498/79-05 50-499/79-05 Appendix A f!0TICE OF VIOLATION Based on the results of the fiRC inspection conducted on April 2-6, 1979, it appears that certain of your activities were not conducted in full compliance with the conditions of your f1RC Construction Permit tio. CPPR-lE8 as indicated below:

Failure to Follow Procedures for Storage of ffatr*ial 10 CFR Part 50, Appendix B, Criterion V requires that activities affecting quality shall be accomplished in accordance with instruc-tions, proceduras, or drawings.

Westinghouse " Technical Manual and Operating Instructions,15 GPM Recycle Evaporator," requires that the Boron Recycle Evaporator be stored in a warm, dry place.

Contrary ta the above:

During inspection of storage areas on April 4, 1979, the IE inspector observed that the Boron Recycle Evaporator stored in the Unit 1 Mechanical-Electrical Auxiliary Building was wet from water which was draining from work areas located above the storage area.

This is an infraction.

Failure to Follow Procedures for Preparation of f!onconformance Repo r_t_

10 CFR 50, Appendix B, Criterion V requires that activities affecting quality shall be accomplished in accordance with approved procedures.

Brown & Root Quality Assurance Procedure ST-QAP-2.6, paragraph 5.1 requires that the description of the nonconforming condition provide sufficient detail establishing the requence of events pertaining to the nonconformance and an accurate physical description of the non-conformance.

Contrary to the above:

On Febuary 9, 1979, as documented on the maintenance records for Residual Heat Removal Pump Motor 2R161tiPA101 A, the motor was found to be wet and the motor terminal connection box was observed to be full of water.

Discussions between the QC inspector who witnessed this condition and the IE inspector indicated this was an accurate description of the motor on February 9,19 9.

On February 14, 1979, flCR S-Ell 37-A and a subsequent t!CR S-Ell 37-B (3-28-79) were prepared with the following description of deficiency:

" Questionable fa gger Reading on Motor for RHR Pump, Equipment e

  1. 2R161f1PA-101 A.

The meg reading is 250,000 ohms."

flo reference was made to the motor terminal connection box being full of water or the possibility of water being in the motor.

This is an infraction.
