" performance of structures, systems, or components that require remedial action or corrective measures to prevent operation in a manner less conservative than assumed in the accident analysis in the safety analysis report..."

D e.s_c r.i.p t i o n of the Event:

The continuing seismic anchor inspection p r o 7. r a m being performed in response to II Bulletin 79-02 identified two shell tyne an cit or s on a four anchor support which failed to torque properly. The emergency cooling water pioing supported by this hanger is placed in service only during usual events such as extreme flood or loss of Conowingo Pond. The operability of the emergency service water system and emergency cooling water system is not diminished during normal opera rions.

The likelihood of a seismic event occurrence coincident with the unusual environmental events is considered to be highly unlikely. Therefore, the safety sienificance of these anchor test failures is considered to be minimal.

Cause of Event:

Investigation indicates the most probable cause of the test failure for these two anchor bolts was improper installation.

Corrective Actionr One anch or b olt (shell type) failed to meet the test criteria and was immediately replaced with a wedge type anchor bolt. In accordance with the approved testin; procedure, the other bolts on this anchs - plate were then torque testad. This identified a second anch or bolt torqr.e test failure. Due to interferences in the area of the second resc failure, twe wedge type anchors were installed on tb. same plate approximat .y 3 1/2" lower than the original bolt locations. This modification of the a3chor was performed in accordance with the inspection program. Corrective action nas been c omp le t e d .

The anchor inspection program, as required by IE Bulletin 79-02, is continuing and will serve to ensure that all seismic anchors are satisfactorily installed.
