On June 8, 1979, Mr. F. R. Hodges of Public Service Company of Indiana, Inc. (PSI) telephoned your office to notify you of a potentially reportable deficiency involving socket welded fittings in the unit I reactor coolant system. Reactor coolant pipe lines, IRC 32AA and IRC 32AB were installed with socket welded fittings using Cherne Contracting Corp. Procedure 13.10, Revision 1.

socket welded fittings.

l'he pipe lines in question are.75 inch lines which connect directly to the reactor vessel head gasket monitoring connections.

The lack of procedural controls over the fabrication and installation of these pipe lines left their quality indeterminate. A reactor head gasket failure coincident with failure of these pipe lines could create a small reactor coolant leak.

These lines have been completely removed by the contractor. Cherne has developed a small pipe fit-up procedure (ISP-001, Rev. 1) rhich was reviewed and approved by PSI on June 15, 1979. The two reactor coolant system lines were completely re-fabricated and re-installed on June 15, 1979 using procedure ISP-001, Rev. 1 for fit-up.

A surveillance of this work was performed by PSI Quality Assurance personnel and no discrepancies were noted. Additionally, the contractor provided training to supervisory and Quality Control personnel on the need for procedures for activities affecting quality.
