On 11-8-78, several attempts were made to start AH-C-8A.

Each time the unit would trip on low lube oil pressure af ter approx-imately 1 minute runr.ing time.

Investigation showed that several loose fittings in the external oil system caused cavitation of the oil pump and low lube oil pressure.

In addition, a faulty overpressure

-trip switch was discovered. After the above discrepancies were corrected, the chiller was started and ran satisfactorily.

PORC has reviewed the above events and determined that it is not reportable because AH-C-8B would have been t.ble to supply both cooling coils. This was demonstrated by the vendor field representative during an inspection of both chiller units.

Further, due to the outside air temperature, operation of the chiller unite was not required to maintain control building component temperatures.

ihe philosophy discussed herein is consistent with discussions held on March 8,1973 with the principle NRC inspector.

- i e' Pressurizer Lede1TTransmitter LT2, which is one.of three transmitters which monitor the same range of the Pressurizer, was declared inoperable and valved out of service on 10-23-78. The problem with LT2 was found to be a lose sensing line fitting which caused the respective transmitter wet reference leg to be incorrect.

Since this is one of three redundant transmitters and one was available from each independent tion-Nuclear Instrumentation power supply it was determined that the requirements of Technical Specification 3.3.3.5 and 3.3.3.6 were satisfied and therefore, this event was not reportable.

While perfoming a nomal reactor shutdown a ratchet trip occurred of Group 5 rods from 100%

withdrawn when Group 5 Motion was started by the Gr 6-7' at 25% interlock.

Power level was less than 1x10-8 amp. After the ratchet trip three rods in Group 5 were stuck at 5% withdrawn.

These rods were freed with the reacter 6.

Subsequent investigation revealed that a field change, designed in code 3.

to prevent ratchet trips, had been improperly installed on one Gr 5 gate This was drive nodule and two auxilliary power supply gate drive modules.
