Operating License No. DPR-73 Docket No. 50-320 Inspection Report No. 78-36 This letter and the attachmeat are in response to your inspection findings letter of January 9,1979, concer:,ing Str. Haverkamp's inspection of T3fI-2.
The results indicated one apparent deficiency.

!acket No. 50-320 nspection No. 78-36 Based on the results of an NRC inspection conducted on December 4-8 and December 12-14, 1978, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-73 as indicated below. This item is a deficiency.
Deficiencv:
Technical Specification 4.6.1.1 states, in part: " Primary CONTAINMENT INTEGRITY shall be demonstrated:
At least once per 31 days by verifying that:
a.
- 1. All penetrations *...are closed by valves...".
- Except valves...which are located inside containment and are locked, sealed or otherwise secured in the closed position.
These penetrations siiall be verified closed during each COLD SHUTDOWN except that verification of these penetrations being closed need not be perf ormed more of ten than once per 92 days.
Contrary to the above, containment penetration isolation valves inside containment were not verified closed during two COLD SHUIDOWN periods on November 10-20 and 23-30, 1978. These valves had not been verified closed since August 26, 1978, during a previous COLD SRUTDOWS period.
Response to Deficiencv:
Technical Specification Surveillance Requirements 4.6.1.1.a.1"*" states that, for certain valves, blind flanges and deactivated automatic valves which are located inside containment, these penetrations shall be verified closed during each cold shutdown (MODE 5) except that verification of these penetrations Su rveillance being closed need not be performed more of ten then once per 92 days.
Procedure 2311-5 incorporates this requirement to verify that containment isolation valves and flanges inside containment are closed, and, in addition, incorporates separate requirements to verify that airlock door seal leakage is within limits. The valves inside containment were verified closed during performance of 2311-5 on August 26, 1978, while in Mode 5.
Th2 plant entered Mode 4 on August 28, 1978 and was subsequently in Mode 5 during November 10-20 and 23-30, 1978.
The air lock door seal leakage portion of 2311-5 had been properly performed throughout this period, but the valve verification portion of 2311-5 was not performed af ter August 26, 1978.
2311-5 was perf ormed at the first cold shutdown follewing discovery of this deficiency.
Appropriate operations personnel will be counselled as to the necessity to perform the applicable sections of 2311-5 to ensure compliance with T.S.
4.6.1.1.a.1"*".
Finally, 2311-5 and 2102-1.1 (Unit Heatup) will be reviewed and revised, as necessary, to clarify which sections of 2311-5 must be completed i g eng.<gr,e,3
