APPENDIX A NOTICE OF VIOLATION Metropolitan Edison Company Docket No. 50-289 Based cr the results of the NRC inspection conducted on December 12-14, 1978, it appears that certain o' your activities were not conducted in full compliance with the conditions of your NRC Facility License No.

DPR-50 as indicated below.

Technical Specification 6.8.1 states in part, that:

" Written procedures shall be established, implemented, and maintained...."

Unit 1 Surveillance Procedures 1300-3B, 3G, and 3I in Step 7.4 of their Acceptance Criteria, state in part, that:

... compare the valve stroke times with those from the previous performance of this test.

If an increase in stroke time of 25% or more from the previous test...is observed, test frequency shall be increased for those valves to once each month until corrective action is taken."

Unit 1 Surveillance Procedure 1300-3H, " Makeup Pump and Valve Functional Tests," Revision 1, March 11,1978, states that:

" Surveillance Frequency - 31 Days (M) for Pump Testing - 92 Days (Q) for Valve Testing."

Contrary to the above, procedures were not followed on eight occasions during the period January 2, 1978 to July 2, 1978 in that:

On seven occasions valve stroke times increased by more than 25 percent from the previous test for valve nur ers (EF-V30 A&B, MS-V138, NR-VlB, NR-VlC, NR-V48, DK-V3) and test frequency was not increased nor was corrective action taken; and, 2.

Surveillance Procedure 1300-3H was not performed from January 2,1978 to July 2,1978, a period of time in excess of 92 days.
