On June 18, 1976, Mr. L. Narrow of your office was verbally nctifiad of n situcti:n --hich Metrcpolitan Edice Cenpany considered to be reportable in a cordance with the requir e.ents of 10 CFR f 0.55 (e). This letter constitutes a thirty-day follow-up letter.

Descricticn As ; art of the construction of the shield walls and slabs around the fuel transfer tubes, holes were drilled into the north cxterior vnll of the Fuel Transfer Canal for reinforcing dcucis.

During the precess of drilling these holes and exposing the rebar, a void was opened within the concrete ever both fuel transf er tubes.

Each void extended about 3 to 4 feet into the cencrete, was approxi-r.ately IS" high and 6' to 8' wide.

The voids were located in a section o.f the wall that had initially been bicckcuts for the subsequent installation of the-fuel transfer tubes. Apparently, when the blockouts were poured, the interior portion of the peur did not fill the keyways on top of the blockouts leaving the recently detceted voids.

On July 20, 1976, Mr. L. Narrew of ycur office was verbally notified of a situation uhich Metropolitan Edison Cc:pany considered to be report-able in accordance with the requirements of ICCr?.50.55(e). This letter constitutes the required thirty day follow-up letter.

Descriotion While performing internal wiring changes on GE AFD-5 switchgear as directed by the engineer, several butt splices located en top of the switchgear for the unit substations pulled apart.

The electricians p.erfor=ing the wiring changes were repairing these splices as they broke. No record was =aintained of the number of splices which required repair.

Descri, tion Gathefing information en required and expected Engineered Scfety Features response ti=es during the preparation of the respense time test progran revealed that several cf the expected response times (based en vender test data, diesel leading sequence, etc.) were in excess of the required ti=es established by FSAE analyses or other considerations.

Analysis of Safemi Inclications If the discrepancies in the required versus expected response times had re=2ined undetected, and if a LCCA coupled with a total loss of offsite power had occurred, the consequences of such an acci-dent conceivably cculd have exceeded those consequences of this accident as calculated in the plant safety analysis, however, preliminar" evaluation indicates that the effect would be slight, adding only about 0.3 psi to the calculated peak containment pressure, and wculd thus have produced onigt--

such an accident en the health c staff.
