7 77 The cbjective of the Fact Finding Ecard is to conduct, as a full-time effort, an in-depth investigation of the accident and to evaluate its origins and consequences from both technical and procedural viewooin'fs.
The investigation should include, as a minimum, careful examination of:
o Pricr events at TMI-2, or any other nuclear pcwer facilities, to determine whether any indications of an impending problem were available before the accident.
s Adequacy of:
design and design criteria, materials of manufacture, installation, quality assurance, and maintenance; o Completeness of safety analyses and review; o Readiness of TMI-2 for ir.itial operations; 9 Training and qualifications of operating and mainterance perscrnel',
s Secuence of events, causes and consequences; e Operating crocedures and management overview, cperater actions; 3 Resconse of licensee and tiRC staff; o fiRC management of situation; e
Federal / State coordination; e Outside influences on tiRC which affected mangement of incident; Emergency plannir.g, response and decision making; e
o Societal impacts; 6 Support needed by, and made available to, both operating personnel and management; C^

The Fact Finding Ecard shall independently pursue this investigation in any manr.er which it deens appropriate.
The Board shall undertake any actions or further investigaticns considered necessary to meet its responsibilities.
The scope of the Fact Finding Scerd will cover all events up to and including placinc the plant in a stable, cold shutdown condition.
Infornation obtained frcm the post-shJtdown period shall be Considered to the extent it bears on the accident and initiating events.
