NRC letter dated June 24, 1975 Inspection 75-07 This letter and its enclosure are in response to your Inspection Report of June 24, 1975, concerning your Mr. Nar-row's June 10 - 13, 1975, inspection of our Three Mile Island Nuclear Station Unit 2, and the findings thereof.

ELR Specificaticn 2555-72, Divicion 13, Secticn 13A, Paragraph 3 1.2.3 states,
" Arc strikes, starts and stcps shall be rezcved by crinding". Par 1 graph 3.1.2.8 of this specificaticn states, in part, "Only cne type of filler =ctal shall be issued to a velder at any one time and any previcusly issued but not used, shall be returned for bake-cut er dispos al".
Contrary to the abcve:
- 1. Welding of safety-related piping was not in accordance with Specification 2555-72
'n that the velder made nurcrcus are starts and steps withcut grinding.
- 2. Control of filler metal was not in accordance with Specificatica 2555-72 in that the velder was cbserved to have more then ene type of filler metal in his pcssession vhile working on safety-related piping.
Corrective Action Tsken Welding of joint F"J-lh8-Al was stepped.
"'he joint was ground back to the veld root and revelded in accord with the specification requirements. The E7018 covered electrode was recoved frc= the centainer and destroyed. The bare filler retal, 308L stainless steel, was cleaned and used to co=plete the velding of joint MU-lh3-AS.

Corrective Steps Taken to Avoid Further Violations On June 13, 1975, the United Engineers & Constructors Uelding Superintendent held a conference with the Pip ng Supervisors, Piperitter Foreman and Pipefitter Welders. Requirecents of the Architect Engineer's Specificatien 2555-72 ere discussed in detail, and personnel vere advised that future violations vill result in with-drsval of velder certificaticu or termination. UE&C Piping Supervisors, Welding Supervisers aa.d Quality Centrol Inspectors vill closely cbserve performance so as to crevent re.:urrence.

Within this series of revicicns, care the requir2 ent fcr actual charpy tect results to be on site. These reviciens were interpreted to =ean that charpy test repcrts vere required for these tank cc=penents which had been reverked and nct those which had already successfully ccapleted the test.
As noted above, QA had reviewed correspondence pertaining to and was aware of the results of the tests perferned en the ncn-revorked portions of the tank prior to the release of any caterial for installation.
_Cc-ective Action Taken The four test reports which were net included in the documentation vere requested frc: Babecek & Wilecx Cc pany. The test reports were received en site the same day they were requested and were reviewed by the Inspector prior to his exit frc= the site.
The additional test reports have been placed in the data package, and the entire package re-reviewed and signed off by Site Quality Assurance.

In order to preclude pessible future ticinterpretations, Quality Assurance persennel have been inctructed to seek additicaal assistance when specification interpretation is required for clarity.
