y Ee.: August 3,197h, Report of Usirg Unapproved Quality Centrol Precedres (1) Description cf the Deficiency:
A periodically issued Status Repcrt of Approved Quality Centrol Procedures was not referenced and therefere nct used to ensure that verk was being done in acccrdance with cnly apprcved precedres. This led to a ecnditien wherein verk was being perferred en the stea: generatcrs and related reacter ccolant piping with unapproved subcentracter Quality Centrol Procedures. This ccnditien is censidered to represent a deficiency in ecnstructica in that there was a "significant breakicvn in (a) pcrtion of the quality assurance progra: (being) ccnducted in acccrdance with Appen-dix 3 (cf 10 CFR 50)."
(2) Safety Implications:
2 e unapproved precedures in questien relate to cleanliness requirements and access centrol, and as a result cf the deficiency described in (1) ateve, it is pessible that tne primary side of the stea: generators and the inside of the adjacent reacter ecolant piping Oculd have tecete cen-ta=inated; which centaH atica could, in t=n, have had an adverse effect en the internals of the steam generatcrs and related reactor ecclant piping.
It has been dete =ined, hcwever, that no such centamination eculd have cec = red in that the tvc une.pproved Quality Centrol Procedr es in questien did not require substantive changes for final approval--the enly changes made vere changes of clarificaticc--se that verk done under the unapprcved precedures was not any different than if it had been done under the ap;reved precedures.

To prevent the recurrence of such a ccnditien, the apprcpriate q'; alit / centrol superviscry perscnnel vere instructed in the proper use of the Status Repcrt Of apprcved Quality Centrol Preced r es.
