License No. DPR-73 APPENDIX A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on February 6-10 and 13-14,1978, it appears that one of your activities was not conducted in fuli compliance with the conditions of your NRC Facility License No.
OPR-73, as indicated below:
This item is an infraction.
Technical Specification 3.9.2 states, in part "As a minimum, two source range neutron flux monitors shall be operating....one with audible indication in the containment.... "
Contrary to the above, during fuel loading on the afternoon of February 13:, 1978, the inspector identified that the existing audible indication in the containment was found to not be effective to alert the operator on duty to meet the intent of the Technical Specifications.
