RESPO: ICE TO INSPECTION REPORT No. 78-00 The following vas included as Appendix A to the above referenced Inspection Report.

Notice of Violation

" Based on the results of the NRC inspection conducted en Febraa-y 6-10, and 13-14, 1978, it appears that one of your activities was not conducted in full ec=pliance with the conditicns of your NEC Facility Iicense No. OPR-73, as indicated belev. This iten is aa infraction.

Technical Specificatics 3.9 2 states, in part, "As a =in"

, two scurce range neutren flux =cnitors shall be cperating,...one with audible indication in the cont ai.=ent..."

Contrary to the above, during fuel loading en the afternocn of February 13, 1978, the inspector identified that the audible indicatica in the centain=ent was fcund to not be effective to alert the operater on duty to =eet the intent of the Technical Specifications."

Corrective Action During cbservation of fuel leading in the Reactm Building, the inspector did not hear the expected clicking frc= the audible source range neutres instrument cutput (Technical Specification 3.9.2). The responsible operater was not aware of the audible "Hovler" on the counter / scaler which was attached to the auxiliary neutron detectors and also could not adequately hear its cutput.

The shift supervisor toch i==ediate re=edial action by adding a model SK-1 clicker to the counter / sealer for additional audible indication and instructed the responsible operator in his duties of continuously monitoring the endible indication.

Cerrective Steus to be Taken to Avtiid Further Violations The refueling procedures vill be revised to ensure that a specific operator inside centain=ent is responsible for continucusly =enitoring audible count rate.
