" Measures shall be established 10CFR50, Appendiz B, Criterion IVI states in part:
to assure that conditions adverse to quality... are pronptly identified and corrected." The accepted Quality Assurance Progran for Startup and Testing
" ANSI N45.2 vill (7SAR Section 17.3), Section 17.3.2, Iten 3 stater in part:
be conplied with..."
ANSI N45.2, Section 17 states in part:
"Measurec shall be established and docu=ented to assure that conditions adverse to quality...
are promptly identified and corrected ns soon as practicable..."
Contrary to the above, measures had not been established and documented nor had conditions adverse to quality been corrected as soon as practicable in that:
- Finding 4 of Audit 76-12 der. ling with an inadequate qualification procedure and a lack of certification of personnel perforning safety related inspections was identified in a letter dated July 13, 1976 and was neither asvered nor corrected an of February 24,

1977; and.
- Finding 8 of Audit 7E-12 dealing with a f ailure to define, in writing, the authorities and re aponsibilities of certain personnel performing activities affecting safety was identified in a letter dated July 13, 1976 and was neither answered nor corrected as of February 24, 1977.
These two findings represent a failure to ansver/ correct approxi=ately 10% of the audit findings conducted under the Startup and Test Quality Assurance Progran which required ansvers/ corrections as of the date of the inspection.
Corrective Action Taken 1.
Finding 4 of GPUSC Audit 76-12 was responded to on February 28, 1977 and subsequently verified and closed on April 5, 1977. Details as follows:
- UE&C Quality Control Procedure QC-29-2, Revision 3 for Ouality control Personnel Traininst and Oualification was revised by Field Change QC-212 on 7-21-76 to require Quality Control Engineers and Inspectors who are responsible for inspections of Startup and Test activities to be quaJ.ified in accordance with ANSI N45.2.6, Rev4.sion 1.

. Corrective Action to Prevent Recur.

GPUSC Chief QA Auditor has issued instructions to the site (Letter No.
THI-II-3984 dated 4-15-77) on the method to be used for resolution of audit findings.
This instruction includes:
1.
The monthly publication of tne Audit Status Se., ries List will include current status identity (in working days) to ansvers of audit findings and reco=endations.
2.
When problems arise in answering or imple=entation of audit findings and reco=endations longer than 30 working days, action vill be taken with Managers / Supervision of the respective organizations by the Chief QA Auditor.
3.
A :. mthly review of the audit status prior to issue vill be made by the Chief QA Auditor and Manager of Quality Assurance to establish additional course of action as might be required on responses and i=ple=entation (that are within the time frames of 30 to 60 working days or longer).
