License No. OPR-73 APPENDIX A NOT' ICE OF VIOLATION Sased on the results of the NRC investigation conducted on March 6 and 15, 1978, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No.

DPR-73 as indicated below.

This item is an Infraction.

10 CFR 50, Appendix B, Criterion V, requires, in part, that

" Activities affecting quality shall be... accomplished in accord-ance with instructions, procedures and drawings."

Paragraph 17.1.5.4 of the Quality Assurance Program documented in the FSAR states, in part, "... all quality related activities on structures, systems and ccmponents within the QA program are per-formed in accordance with specification, procedure and drawing requi rrmen ts. "

Section I of United Engineers and Constructors' QC Procedure QC-BA-2, Revision 2, for welding control states that this procedure applies to welding, exclusive of pipe welding performed on nuclear and nuclear related areas and on Class I seismic structural building members in the reactor building.

Paragraph A-4.a of Section IV of this procedure states, in part, " Prior to welding, verify that approved welding procedures... are to be employed in the work."

Contrary to the above, on March 15, 1978, UT examination of studs used for attachment of the west anchor plate for the structural support of Core Flood Tank, CF-T-1B, and failure of the licensee to provide an approved welding procedure for welding of the studs, substantiated an allegation that _ welding of these studs had been performed without approved welding procedures.
