NRC-RES/EPRI FIRE PRA METHODOLOGY:

Task 12 - Fire HRA EXAMPLES OF HUMAN PERFORMANCE IN REAL FIRE EVENTS NRC-RES Fire PRA Workshop Module IV August 5-9, 2019 Rockville, MD

Agenda for Fire Event Discussion Retrospective analysis of operational events

- Why?

- How to do?

- When/where to use results?

Example fire events for discussion:

- Browns Ferry

- Waterford 3 (maybe)

Classroom exercise in retrospective analysis of fire events

- H.B. Robinson Fire HRA - Examples of Human Performance in Slide 2 Fire PRA Workshop 2019, Rockville, MD Real Fire Events

Why perform retrospective analysis of events (fire included)?

There are many reasons why review and analysis of past operational events is useful and/or important For HRA purposes, these reasons might include:

- To understand the operational characteristics and challenges for a new technology or new application, e.g.,

fire events in NPPs, normal operations at a chemical weapons destruction facility, spent fuel handling in NPPs To be updated or reminded of current operational challenges (serving as a seed for information collection in HRA qualitative analysis and HFE modeling)

Why perform retrospective analysis of events?

(continued)

Note that, here, we are focused on milking documentation of operational events for HRA insights NOT performing HRA to support Accident Sequence Precursor (ASP) or other similar analyses Fire HRA - Examples of Human Performance in Slide 4 Fire PRA Workshop 2019, Rockville, MD Real Fire Events

How to do retrospective event analysis for HRA insights?

At a high-level, answer following questions:

-What happened?

-Why?

For example:

- what are key factors determining accident progression?

- What are key factors influencing operator successes and failures?

How to do retrospective event analysis for HRA insights?

(continued)

NUREG-1624, Rev. 1 (ATHEANA) includes a retrospective event analysis approach

- Chapter 8 briefly describes the approach

- Appendix A provides illustrative examples Because the purpose is to obtain HRA insights, we want to collect and present information with HRA/PRA applications and operations perspectives in mind, such as:

- Initial and final plant conditions

- Initial and final status of safety systems

- Hardware and instrumentation failures

- Timeline of events

- Key operator actions (or inactions) that might be modeled in a PRA Fire HRA - Examples of Human Performance in Slide 6 Fire PRA Workshop 2019, Rockville, MD Real Fire Events

When/where to use results?

HRA insights from retrospective event analyses have several uses, such as:

- Develop an understanding of a new PRA hazard, e.g.,

Unpublished ATHEANA retrospective analyses of fire events were used to develop original list of PSFs for NUREG/CR-6850

- Develop understanding of a new technology

- Develop understanding to base a new HRA method

- Seed analysts in performing HRA

- Seed discussions with operations staff when performing HRA Stories can trigger more stories and memories that can assist analysis Fire HRA - Examples of Human Performance in Slide 7 Fire PRA Workshop 2019, Rockville, MD Real Fire Events

Example fire events for discussion Worked out illustrative examples of retrospective analyses of fire events:

- Browns Ferry

- Waterford 3 (maybe)

Briefly review event description Discuss key information captured in event Refer to separate Word documents for event analyses Fire HRA - Examples of Human Performance in Slide 8 Fire PRA Workshop 2019, Rockville, MD Real Fire Events

Classroom exercise in retrospective analysis of fire events Class will be broken up into groups Each group will use provided template for retrospective analysis Each group will develop analysis of H.B. Robinson fire event using:

- AIT

- ASP analysis Refer to references provided on training website Fire HRA - Examples of Human Performance in Slide 9 Fire PRA Workshop 2019, Rockville, MD Real Fire Events
