Pacific Pump Division Docket No. 99900063/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on October 1-5, 1979, it appears that certain of your activities were not conducted in accordance with h7C requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these l
instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Deviations from these requirements are as follows:
A.
Chapter 7 of the Quality Assurance Manual, Paragraph 7.3.3, states in part, ".
. Coated electrodes which remain unused after their container has been opened are kept in holding ovens, each like heat, size and kind carefully separated from others.
1 Contrary to the above, coated electrodes were not carefully separated from others by size in that 5/32 and 3/16 7018 coated electrodes were observed mixed in one ecmpartment in holding oven No. 11.
B.
Chapter 5 of the Quality Assurance Manual, Paragraph 5.6, states in j
part, ".
It is evident that work covered by any of the above documents cannot proceed until the document has been released for use.
." The Summary of Processes (SP) is listed as one of the above documents.
Contrary to the above, work covered by the Summary of Processes document SP-BN50479-1 for Order No. BN50479, Item A03F, operation 40, weld on I
hold down blocks, had been accomplished although the Summary of l
Processes which defined the welding procedure for operation 40 had not been released for use.
