APPENDIX A NOTICE OF VIOLATION Consolidated Edison Company of New York, Inc.
Docket No. 50-247 Based on the results of an NRC inspection conducted on April 29 - August 18, 1979, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Facility License No. DPR-26 as indi-cated below.
Item I is a deficiency, and Item II is an infraction.
I.
Technical Specification 6.8.1 states, in part, " Written procedures and administrative policies shall be established, implemented and maintained..."
Station Administrative Order No. 126, Jumper Log, requires that each jumper issued shall be logged in a " Jumper Log."
Contrary to the above, a jumper installed in the bearing lift pump control circuit on June 19, 1979 was not entered in the Junper Log.
II.
Technical Specifications Section 4.4.IV states, in part that, " Isolation valves which are pressurized by the Penetration and Weld Channel Pressuriza-tion System will be leakage tested as part of the Sensitive Leakage Rate Test."
Contrary to the above, the leakage rate for valve No. PCV 1191, which is the first valve outside containment on the containment building pressure relief line, has never been established.
