5'0-556/79-04 50-557/79-04 Appendix A NOTICE OF DEVIATION Based on the results of the NRC inspection conducted on October 2-5, 1979, it appears that certain of your activities related to Black Fox Station, Units 1 and 2 deviate from commitments made in your Preliminary Safety Analysis Report (PSAR) as indicated below:
Section 17D.1.5 of the PSAR states that all Field Project Management activities that affect quality of safety-related items will be per-formed in accordance with documented proccdures.
Procedure FPPM 4.8, " Storage and Preservation of Material and Equipment,"
Revision 1, requires that Equipment Storage Maintenance Cards (Form B-3038) shall be kept in plastic envelopes attached to items requiring preservation and maintenance.
Contrary to the above:
An unapproved form, " Preventative Maintenance Record," was substituted in place of Form B-3038 for a 16 inch gate valve (MPL E 22F001) stored in Warenouse C.
This is a deviation.
