The issuance of this report was delayed pending a meeting with the
corporate management of this facility, which was not scheduled until
,
August 8, 1972 and concluded on August 9, 1972.
This inspection was made to determine if the QA program inadequacies
i
could be related to the implementation deficiencies, particularly in
concrete placement.
.
The QA program deficiencies do not appear to be of such a nature
!
that could result in the deficiencies which have occurred, except for
the fact that QC personnel did not appear to be aware of their author-
,
ities.
,
An inspection to determine the adequacy of implementation was conducted
I
on August 14, 1972 and will be forwarded at an early date.
Please note that Sections II, III, and IV were consolidated due to the
nature of the items inspected.
