We vould like to point out that no member of the public and no station or contractor personnel received a radiation dose in excess of the limits stated in 10 CFR 20 as a result of the release. Further, none of the limits given in the Technical Specifications were exceeded.

NOURCUTDZ 10-DAY REPORT 75-10 REPCRT OF AN U'iPlA:CIED RELEASE OF RADICACTIVE MATERIAL CCCURRDG ON DECE:EER 6,1975 ,

f Descrittien of Occurrence:

On December 6,19'is, between the hotrs cf 10h5 and 1245 (two hours),

an inadvertant release of radioactive material occurred while water was being added to the Reactor Coclant System from Bleed Tank "A". This addition of water to the Makeup Tank resulted in an increase of pressure in that tank which caused the release of radioactive material thrcugh either the Makeup Tank manvay gasket or frcm a valve (MU-V13) isolating the Makeup Tank from the Vent Reader.

Arrarent Cause of the Occurrence:

No apparent cause has been definitely identified at this time.

Sampling in the area of the Makeup Tank and MU-V13 during makeup to the Reactor Coolant System is continuing in an attempt to identify any leaks.

Analysis of the Occurrence:

For the following reasons it is believed that the unplanned release of radioactive material en the 6th of December did not endanger either the health or safety of the public,

a. None of the limits in the TMI-l Technical Specifications were exceeded.
b. None of the maximum permissible concentration limits _'or non-radiation vorkers as given in 10 CFR 20 were exceeded at the site bounde.ry,
c. No individual on site at the time of the release received a radiation dose in excess of the limits for radiaticn verkers specified in 10 CFR 20.

Corrective Action:

The release was identified subsequent to the makeup evolution. The imediate corrective action was to sample the Makeup Tank rocs to verify the source of leakage.

A program has been initiated to identify problematic areas in our Vent Header System and cor-ect any deficiencies. As part of this program, a special operating procedure was written to require sa=pling of the Makeup Tank roca and the i=edint area adjacent to MU-713 during a makeup to the Reactor Coolant System in an atte=pt to isolate the source of leakage.

NOTE: In no case did any member of the public cr any station er con-tractor personnel receive a radiation dose near the applicable limits given in 10 CFR 20.

The total release consisted of .262 curies of predeminantly Xe-133 (assumed to be 100%) based on Radiation Monitoring System Strip Chart Recordings. The maximum instantaneous noble gas release rate during the two (2) hour period was 2.68 x 102 M3/see which is below the Technical Specification limit of 1.2 x 105 M 3 /sec. The average release rate during the period was 1.21 x 102 M3 /sec. There vere no personnel exposures involved in this release since there ve e no cperational or maintenance functions evolving in the Reactor Ccolan , Makeup Tank Rocs er the area adjacent to MU-V13 during the time of the . elease. Sa=ples taken in the Makeup Tank Room shortly after the release oce';.rred shoved no ga=ma emitters present.
