APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-352 This refers to the inspection conducted by representatives of the Region I (Philadelphia) office at the Limerick Generating Station, Unit 1, Limerick, Pennsylvania, of activities authorized by NRC License No. CPPR-106.
During this inspection, conducted on April 10-12, 1979, the following apparent item of noncompliance was identified:
Appendix B of 10 CFR 50, Criterion V, states, in part: "Ac tivities aff ecting quality...shall be accomplished in accordance with these instructions, pro-cedures or drawings."
The Limerick PSAR, Appendix D, Quality Assurance Program, Paragraph D.6.4, states in part, that: "Bechtel Construction Department...is responsible for construction of the plant to approved engineering specifications, drawings, and procedures...."
Bechtel Power Corporation job rt?le for welding 8031-HR-G-16 states, "Do not veld or burn unless all exposed material is adequately protected. Particular attention shall be provided the area below your weld."
Contrary to the above, on April 12, 1979, a primary steam isolation valve (B21-1-F022) which is located just below a feedwater pipe joint in the process of being welded, was not adequately covered to protect it from the welding activities. The valve was subjected to weld slag, weld grinding dust and dirt.
