a APPENDIX A NOTICE OF VIOLATION Item of Noncompliance Appendix B of 10 CFR 50, Criterion V, states, in part: " Activities affecting quality...shall be accomplished in accordance with these instructions, procedures or drawings."

The Limerick PSAR, Appendix D, Quality Assurance Program, Pa zagraph D.6.4, states, in part, that: "Bechtel Construction Department...

is responsible for construction of the plant to approved engineering specifications, d.rawings, and procedures..."

Bechtel Power Corporation job rule for welding 8031-HR-G-16 states, "Do not weld or burn unless al.1 exposed material is adequately pro-tected. Particular attention shall be provided the area below your weld."

Contrary to the above, on April 12, 1979, a primary steam isolation valve (B21-1-F022) which is locat 'd just balow a feedwater pipe joint in the process of being welded, wt not adequately covered to protect it from the welding activities. TL 3 valve was subjected to weld slag, weld grinding dust and dirt.

Response to Item of Noncompliance The subject valve was cleaned and inspected and found to be in an acceptable condition and then the valve was adequately recovered.

In order to avoid further items of noncompliance, the welding Quality Control discipline held a training session to fr.miliarize discipline Quality Control Engineers with the requirements of job rule 8031-JR-G-16. Additionally, the Project Superintendent issued a memo to all responsible Superintendents to remind them of the requirements of the job rule.
