Letter from Mr. G. H. Smith Mr. H. V. Lichtenberger, dated August 9, 1979; Inspection 70-1100/79-04 Gentlemen:

This is in response to the above referenced letter in which you reported three infractions that were determined during your inspector's visit to our facility on June 5-8, 1979.

Infraction #1 Appendix A, Item A

1) Alpha survey instruments were not available for use by personnel at the chang. i#ne at the emission spectrograph located in Building #5 and at the exit to Building #6 to enable personnel exiting these areas to survey for radioactive contamination.

Response

An alpha survey meter has been installed at the exit from the emission spectrograph in Building #5. The requirement and necessity for monitoring prior to leaving this area has been emphasized to laboratory personnel.

We are in full compliance at this time.

Building #6 is locked and personnel access to the contaminated area is per-mitted only under the direction of Health Physics personnel.

All entrances to the contaminated area are posted accordingly.

The instrument normally present at the exit had been removed for repair at our electronics labora-tory. The repaired instrument was in the Building #17 Health Physics office and was returned during the day in which the inspector was escorted through Building #6.

In the event that this instrument is out for repair in the future, a similar instrument will be brought to Building d6 when personnel access to the contaminated area is necessary.

Repair and calibration of the Building #6 survey meter is normally completed within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

2) Our internal procedure "0S 1580 Rev. 4 dated 5/24/73" stated that the predry airflow velocity for the UO2 powder blendirg operation should be "per engineering instruction".

No written instruction was available to the operator at the time of the inspection.

Response

The above procedure was revised and reissued on 8/14/ 9 to include specific 7

instructions regarding predry airflow velocity. Ile ere in full compliance at this time.

Infraction #2, Appendix A, Item B

1) An operator was found to be handling contaninated containers at the powder blending station lift without wearing gloves.

Response

The operator was immediately instructed to put on gloves and w-s reprimanded for this action.

2) An operator was found to be handling wet, unclad uranium fuel pellets at the centerless grinder with cloth gloves when rubber gloves are required.

Response

The operator was immediately instructed to put on rubber gloves and was rep-rimanded for this action. The requirement of wearing cloth gloves when hand-ling unclad dry U0g or contaminated equipment and containers was emphasized at a training session for all pellet shop operators held on 7/10/79.

The re-quirement of wearing waterproof gloves when handling damp or wet U02 was also emphasized.

We are presently in full compliance.

Infraction #3, Appendix A, Item C It was found that sone fuel handling procedures had not been approved in writir.g by the Nuclear Licensing & Safety Supervisor before issuance.

Response

A comprehensive review of all fuel handling procedures and revisions was performed and the required approval signature by the Nuclear Licensinq S Safety Supervisor was added where necessary.

The requirement for approval of all fuel handling p o-cedures and revisions by the Nuclear Licensing & Safety Supervisor before issuance was emphasized by management to all groups responsible for generating these pro-cedures. We are in full compliance at this time.
