Review of Pctential Problems with Surveillance Procedures and Multiple Failures The following infer =ation is supplied in respense to Reference (b).
In the 18 year operating history of Yankee Rcwe cnly a few challenges to the Reactor Trip and Safeguards Systems have cecurred due to cperator errer.
Equipment failures as described in Reference (b) have not occurred during these challenges.
The Plant Operations Review Committee has reviewed the letter in relation to conduct of operatiens at the plant.
Today, as in the past, the plant's policy has been to ccrrect potential problems as they cccur and to correct proble=s before they occur. The con =ittee reviews procedures to =inimi::e the chances of having an occurrence as described, and when a problem develops, the Cc=mittee takes steps to prevent a recurrence.
Manage =ent policies and procedures assure that equipment failures in safety related syste=s are pursued. The Plant C'perations Review Cc=mittee reviews any failure that results in a Licensee Event Report or that leads to a reducticn in the safety syste= ability to functicn as required. The Cc==ittee also reviews failures in safety related syste=s to deteruine if plant safety is significantly reduced.
