Appendix A gl';' 2 7 Y373 NOTICE OF VIOLATION Dairyland Power Cooperative Docket No. 50-409 Based on the inspection conducted on July 25-27, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. These items are deficiencies.
1.
Technical Specification (TS) 3.5.1.5 requires a quorum of the Operations Review Committee (ORC) shall consist of a chairman and six (6) members including alternates.
Contrary to the above, on May 2, 1979, the ORC met to conduct TS required business with only four members in attendance.
2.
Techd cal Specification 3.8.2 requires each procedure and admir strative policy of 3.8.1 and changes thereto, shall be reviewed by the ORC and approved by the Plant Superintendent prior to imple-mentation and every two years thereafter.
Contrary to the above, Figure 2.4.1 of procedure 2.4, " Predicted Critical Control Rod Position" used for startups in May, June and July 1979, had not been reviewed or approved.
