ENCLOSURE SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2 FAILURE TO INSTALL CABLE GUTTER VIOLATES SEPARATIONS CRITERIA NCR 1780 10 CFR 50.55(e)
FINAL REPORT Description of Condition A cable gutter which appears on the design drawings was never installed.
The purpose of this cable gutter is to provide an enclosed wireway between redundant cables where a three-foot separation does not exist in the cable spreading room. The cable gutter was supposed to house class lE cables from main control room panel 0-M-27A to cable trays in the cable spreading room. The as-installed configuration has cables identified as train A routed closer than a three-foot distance to train B cables. Without the cable gutter, divisional separations requirements are not met. There are 119 safety-related cables that should have been installed in the cable gutter.
Safety Implications Had this condition gone uncorrected, a fire could have possibly damaged both trains of safety-related cables. However, since control of equip-ment required for safe shutdown is available in the auxiliary control room, the operation of the plant would be affected but safe shutdown would still be possible. Therefore, this condition could have reduced the redundsucy of control of safety-related equipment.
Corrective Action The affected cables will be enclosed in a metal wireway in order to meet the required separations criteria. This corrective action will be completed before fuel loading of unit 1.
This condition is an isolated occurrence at Sequoyah Nuclear Plant and no other TVA nuclear plant is affected.
