Docket No. 99900262/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on June 18-21, 1979, it appeared that certain of your activities were not conducted in full compliance with NRC requiremetas as indicated below:
Criterion V of Appendix B to 10 CFR 50 and paragraph NCA-4134.5 of Section III to the ASME Code states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with the instructions, procedures, or drawings.
Paragraph 5.0 of Section No. 5 of the ASME accepted Quality Assurance Manual states in part, ".
. All procedures applicable to CCI shall be prepared by personnel knowledgeable in the subject in the appropriate responsible section of CCI. Review and approval of the procedure by other applicable sections and Quality Assurance shall be obtained prior to release of the procedure."
Contrary to the above, the following procedures had not been reviewed and approved by Quality Assurance prior to their release:

0405-01, Design Review for New Products and/or Generic Design Changes for Retrofit.
2.
0405-02, Preliminary Design Review.
3.
DS-101, Revision 0, Pressure Boundary Design Standards.
4.
DS-701, Seismic Proposal Guidelines.
