Public Service Company of Colorado Fort St. Vrain Nuclear Generating Station Docket No. 50-267/79-12 NOTICE OF VI0IATION Based on the results of the NRC inspection conducted June 12-15, 1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Technical Specification AC 7.4.a. requires in part that, " Written procedures shall be established, implemented and maintained...
OPOP I, Section B, General Plant Requirements states in part that, "All system check lists shall be completed in their entirety before startup following a shutdown for refueling..
The instrumentation and control system signoffs, prerequisites per S0P signoffs, and the SLO signoffs are still required."
Contrary to the above, OPOP I pages 11 and 12 were signed off on June 12, 1979 as " Reserve Shutdown System Operable" based on Section B.2.a(1) 1.3 "Preoper-ational Valve lineup made per S0P 12-05" and Section B.2.a(1) 1.4 " Prerequisites complete per S0P 12-05" even though valve liv-1104-1 on page 11 and valve HV-1102-1 on page 12 of appendix 1 to S0P 25 had apparently not been checked for valve lineup.
