Based on the results of the NRC inspection conducted on December 12-15, 1978, it appears that three of your activities were not conducted in full compliance with conditions of your NRC Facility License No. CPR-73 as indicated below:
A.
Appendix A to the License, in paragraph 6.8.1, states:
" Written procedures shall be established, implemented and maintained..."
Licensee Pmcedure AP-1034, in paragraph 6.3, states:
"Should it become necessary to exceed the limits (of transien combustible material) for brief periods, the cognizant department head... must quslitatively evaluate any additional portable or temporary fire protection measures which must be taken in the area... while the limit... is exceeded, the area must be manr.ed, or a fire watch patrol... at least once per hour be estaolished."
Contrary to the above, empty cardboard boxes were stored in the cable spreading rocm, recording paper was stacked on temporary tables there, and a hot soldering iron was slugged into an outlet near the recording paper; i.owever, the area was not manned nor had a fire watch patrol been established.
This item is an Infraction.

Aapendix A to the License, in paragrach 4. 7.10.1. 2 ( a ) 2 s ta tes :
"Each fire pump diesel engine shall be demcnstratec
- rabie at least once per 31 days:...The diesel starts from ameient conditions and cperates for at least 20 minutes."
License Procedure AP-1010, in paragraph 6.6.1, states:
"A retest ir required when initial test results fail to meet the acceptance

criteria or when the test could not be entirely performed.
Retest results will be... evaluated, along with the original data sheets, by the Test Coordinatcr..."

Contrary to the above, a retest of steps 3.a, b and c of Surveil-lance Report 3303-M1, dated September 18, 1978, was not perfomed when the initial test failed, and the test coordinator did not sien the fmnt page of the surveillance report to indicate that he had evaluated the test results.
This item is a Deficiency.
5 C. to the License, in paragraph G.9, states:
"By July 31, 1978:
Provide additional fixed sealeJ beam emergency lights to facilitate emergency operation at remote shutdown panels and faci l i ti es... ".
Contrary to the above, additional emerst.acy lights had not been installed in the cable-spreading room to facilitiate emergency operation of the remote shutdown panel.
