J. E. Konklin, G. F. Maxwell and F. C. Hawkins during the inspection conducted at Perry Nuclear Power Plant from May 22 through 24, 1979 Attached to this letter is our response to the one (1) Item of Noncom-pliance described in Appendix A, Notice of Violation, of the above referenced I. E. Report. This response is in accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice", Part 2, Title 10, Code of Federal Regulations.

RESPONSE TO EITFORCEMENT ITEMS

, Below is the response to the noncompliance identified in Appendix A, Notice of Violation, enclosed with United States Regulatory Co= mission I. E. Report Number 50-440/79-05; 50-441/79-05 I. A. Infraction 10CFR50, Appendix B, Criterion X, requires that a program for inspection of activities affecting quality shall be established and executed by or for the organization performing the activity to verify conformance with the documented instructions, procedum s, and drawings for accomplishing the activity.

The HTPP PSAR Chapter 17, Section 17.1.10 states in part that,

" Quality will be controlled by inspection or process control or a combination of both as necessary. . ."

Contrary to the above, the NRC inspector observed two separate instances in which licensee personnel failed to execute the program for inspection of activities affecting quality to verify confor-mance with documented instructions and procedures:

1. Immediately prior to concrete placement, the following specific conditions adverse to quality had not been corrected in concrete placement No. AX2-WO5A-650. The pour area had been inspected and released for concrete placement by responsible QA/QC per-sonnel in accordance with NECC Procedure QP 10.1, Rev.13
a. Contaminated reinforcing steel,
b. General unacceptable leanliness of the horizontal con-struction joint.
c. Embedded duct tape in the vertical keyed construction joint.
2. A total of five separate concrete placements over a period of two days were not being cured as required by Specification SP-201 and ACI 301-72.

B. Response

1. a. Inspection of concrete preplacement activities has been strengthened by: increasing the Construction Quality Control (CQC) manpower from four to six inspectors; cher.ging CQC inspections from monitoring contractors' QC activities and documentation checks to performance of inspections in-dependent of contractors' QC; and conducting indoctrination and training session for one contractor to e=phasize the level of cleanliness expected prior to concrete placement.
b. Inspection of concrete curing has been strengthened by:

increasing CQC surveillances/ inspections of concrete curing and perfoming Construction Quality Engineering (CQE) audits on contractors' curing activities.

2. a. Since the NRC inspection, only two CQC preplacement inspections have documented problems not previously identified and being corrected by the contractors. CQC is continuing to monitor contractors' preplacement activities to assure continued com-pliance,
b. Inadequate curing conditions noted during CQC surveillances/

inspections and (RE audits were brought to the contractors' attention and immt diately corrected. CQC is continuing to monitor concrete curing conditions. Frequency of CQC sur-veillances/ inspections and CQE audits will be increased if adverse trends in curing activities are detected.
