RESPONSE TO INFRACTION

- NRC COMMENT LISTED IN IE INSPECTION REPORT NOS. 50-280/78-4 A...~'D 50-281/78-4 Based on the results of the NRC investigation conducted on January 20 and 24-25, 1978, it appears that certain of your activities were not conducted in full compliance with NRC regulations and License conditions as indicated below.

These items have been categorized. as described in our correspondence to you dated December 31s 1974.

Criterion IX of Appendix B to 10 CFR 50, as implemented by Vepco Topical Report Quality Assurance Program Operations Phase VEP-1-A, Section 17.2.9, requires, in part, that measures shall be established to assure that special processes, including welding, ....* are control-led and accomplished by qualified. personnel ***.**

Contrary to the above requirement, welding operations were performed by nonqualified personnel during welding of the diaphragm. to body seal weld on the Reactor Building Spray Cooler for Unit No. 2 (Cooler No. 2-RS-E-lB) on November 21, 1977.

This is. an infraction.

RESPONSE

e The infraction listed above is correct as stated.

1. Corrective steps which have been taken and results achieved.

The investigations, review and conclusions of an independent VEPCO investigating. team substantiated the allegation (infraction) as stated. The conclusions of the VEPCO team and the Region II investigations, aft,ar review of safety implications, reveal, "The

-quality of the weld has no safety implications.II

  • II. Corrective steps which will be taken to avoid further non-compliance.

The following corrective steps. have been implemented and should pre-clude non-compliance at a later data.

(A) All Surry Power Station welders have been instructed/reinstructed in the use of the following:

(1) The methodology to use in interpreting the station welder qualification matrix (chart) which is available to all welders and is presently installed in Foreman's office.

(_2) Co.rporate/Station administractive policies concerning welding and the conduct of business in the welding discipline as it is applied at Surry Power Station.

e e (3) All welders have attended a general welder training session which was conducted to clarify possible misunderstood or m~sinterpreted areas on how business is to be conducted in these areas.
