NRC IE BULLETIN NO. 77-02 October 28, 1977 "POTENTIAL FAILURE MECHANISM IN CERTAIN WESTINGHOUSE (W) AR RELAYS WITH_ LATCH ATTACHMENTS" NO. 1 AND 2 UNITS SALEM NUCLEAR GENERATING STATION In response to your letter of September 12, 1977 transmitting NRC IE Bulletin No. 77-02, we have investigated this m~tter with respect to applicability of Westinghouse Nuclear Service Division Technical Bulletin NSD-TB-77-10 criteria to Salem No. 1 and 2 Units.

The following summarizes the results of our investigation.

Salem No. 1 Unit

1.

All AR relays and replacement _car_tridge __.as_.sembl-ies--were

  • -r.~moved. from the station. -storeroom and inspected by station Quality Assurance in accordance with Westinghouse Technical Bulletin NSD-TB-77-10.

All were found to have been manu-factured after.the mid 1973 cutoff date and are therefore rejectable.

Rejected components were tagged with Q.A. _Hold Tags and placed in the Hold Area awaiting receipt of modification kits from Westinghouse.

A Work Order has been initiated (PD-3807) to ver~fy correct relays are installed in the plant.

This inspection is presently in progress and will be completed by December* 1, 1977.

Approximately 90% of the installed relays have been

lO 77 inspected and found to be of the acceptable design.

Documentation of the inspection results will be on file at the station by December 1, 1977.

Inventory cards for an AR relays and cartridge assemblies were removed from file and a caution bote was added stating that any items received on the three subject folio numbers shall be inspected by a Performance Supervisor ih accordance with the Technical Bulletin before they are placed in stock.

Salem No. 2 Unit

1.

Our review of the manufacturing dates indicates that all*AR relays installed in Salem Unit No. 2 were manufactured* prior to June 1973 and do not contain the potential defective latch mechanism.

There are no spare AR relays on the Unit 2 construction site other.than previously examined installed spares.

Documentation of the review results are on file* in the Site Engineering Office.

Our personnel have. been made aware of the potential problem with Westinghouse AR relays and have been instructed to not purchase any additional AR relays until--this problem is resolved.

On future applications for AR relays we will use only thos~ relays manufactured prior to June 1973 or relays modified to resolve the problem.
