LICENSE.NO. DPR-70 DOCKET NO. 50-272 REPORTABLE OCCURRENCE 76-16/3L Pursuant to the requirements of Salem Generating Station Unit No. 1 Technical Specifications, Section 6.9.1, we are submitting Licensee Event Report for Reportable Occurrence 76-16/3L.
This report is required within thirty (30) days of the occurrence.
This report replaces Reportable Occurrence 76-16/lP which was incorrectly determined to be an LER and inadvertently reported on December 1, 1976.

Improper use of test leads.
CONDITIONS PRIOR TO OCCURRENCE:
Mode 3 operation, Plant pressure 2235 psig, Plant temperature 547°F.
DESCRIPTION OF OCCURRENCE:
On 12/3/76, a safety injection signal was initiated due to Hi Steam Line Differential Pressure.
I&C personnel were in the process of connecting a brush recorder to No. 12 Steam Generator Pressure Channel III. (Details of the ECCS actuation will be contained in the 90-day special report sub-mitted.pursuant to Technical Specification 6.9.2(a).)
The Shift Supervisor implemented EI I-4.2, Recovery From Inadvertent Safety Injec-tion, and plant conditions were returned to normal.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The cause of this occurrence was the use of grounded type test leads rather than the required ungrounded type.
All brush recorders use the ungrounded type test leads which are tagged "For Brush Recorder Only".
A Technician attempted to use grounded visicorder leads which were not tagged.
ANALYSIS OF OCCUR~NCE:
Operating pressure and temperature was 2235 psig/537°F before and 2260 psig/539°F after the safety injection.actuation which is within the limitation of the reactor coolant system.
With the Boron Injection Ta.nk boron concentration less than that requires by S.T.S. paragraph 3.5.4.l(b)
S. T. S. Action. Statement 3. 5. 4.* 1 was implemented.
No damage occurred to the reactor coolant system or the emergency core cooling system and at no time was there any hazard to the general public or site personnel.
CORRECTIVE ACTION:
All brush recorders have been modified to use the grounded type leads which are interchangeable with visicorders.
An inter-shop letter is being circulated among all I&C Technicians and Supervisors requiring them to initial.it, signifying knowledge of the changes made.
FAILURE DATA:
This is the first occurrence of this type.

Corrective action has been taken to modify the brush recorders to use any type of test~_leads. Recorders have also been marked and color coded.
This was the first occurrence of this type. (76-16/3L).
CAUSE DESCRIPTION (Continued) attempted to use. grounded "Visicorder" leads which were not tagged.
All brush recorders have now been modified to use the grounded type leads which are now interchangeable with the visicorder.
- An inter-shop letter is being prepared that discusses this change.
This letter will be circulated among all Technicians and Technical Assistants requiring them to initial it, signifying knowledge of the changes made.
