- 13 Auxiliary Feed Pump fail to start.
CONDITIONS PRIOR TO OCCURRENCE:

2235 psig DESCRIPTION OF OCCURRENCE:
Reactor critical at 30% power when No. 12 Steam Generator Feed pump tripped due to low suction pressure.
Loss of No. 12 Stearn Generator Feedwater Pump resulted in low levels in Nos. 13 and 14 Stearn Generators causing a Reactor and Turbine trip.
Low levels in two of the four Steam Generators also initiated an automatic start signal to Nos. 11, 12 and 13 Auxiliary Feedwater Pumps.
No. 13 Auxiliary Feedwater Pump failed to start.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The cause of this occurrence was personnel error.
Investigation revealed that the Auxiliary Feed Pi..nnp Turbine was manually tripped.
A test was run simulating low level in two of the four Stearn Generators and No. 13 Auxiliary Feed Pump controls functioned correctly and the pump started as required.
Check-off sheet 4.1 of OI III-10.3.1 verifies the Auxiliary Feedwater System line-up after the system is secured when escalating power.
This check-off was overlooked and not run.
The manually tripped turbine was not identified.
The No. 13 Aux. Feed Pump was last demonstrated operable on 12-29-76 during surveillance 4. 7.1. 2 (a) (1).

Technical Specification 3.7.1.2 allows 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> to restore an inoperable Auxiliary Feedwater Pump.
No. 13 Aux. Feed Ptimp was restored to operable status well within the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> allowance.
Nos. 11 and 12 Aux. Feedwater Pumps started normally and were available throughout this occurrence.
The limiting condition for operation was not violated.
At no time was there any danger to the general public or site personnel.
CORRECTIVE ACTION:
All operato~s will be reinstructed on the requirements of or III-10.3.l and the importance of verifying the Auxiliary Feedwater line-up.
FAILURE DATA:
Component failure is not applicable to this occurrence.

All Turbine controls functioned properly and No. 13 Aux. Feedwater Pump started normally.
Check-off sheet 4.1 of 01 III-10.3.1 requires the Aux. Feedwater System line-up to be verified after Aux. Feedwater is secured.
Check-off 4.1 of OI III-10.3.1 was not run after securing the Aux. Feedwater System and the manually tripped Turbine was not identified.
The No. 13 Aux~
Feedwater Pump was last demonstrated operable on 12-29-76 during the performance of Surveillance Requirement 4.7.l.2(a)(l).
