SUMMARY

The United States (U.S.) Department of Homeland Security's (DHS) Federal Emergency Management Agency (FEMA) Region IX National Preparedness Division (NPD), Technological Hazards Branch (THB), evaluated a Medical Services (MS-1) exercise for emergency Off-site response organizations (ORO) on August 22, 2018, at Marian Regional Medical Center (MRMC),

Santa Maria, California.

The predesignated MRMC is located outside the Emergency Planning Zone (EPZ) surrounding the Diablo Canyon Power Plant (DCPP).

The purpose of the exercise assessment is to assess local ORO level of preparedness in response to a radiological incident occurring at DCPP. The exercise assessment is part of the Radiological Emergency Preparedness Program (REPP) to ensure that adequate capabilities exist to prevent, protect against, mitigate the effects of, respond to and recover from radiological incidents involving commercial nuclear power plants (NPP).

The MRMC MS-1 exercise, was held in accordance with FEMA' s policies and guidance concerning the implementation of State and local ORO Radiological Emergency Response Plans (RERP) and procedures.

The Extent-of-Play (EOP) Agreement was reviewed, and approved by the FEMA Region IX Regional Assistance Committee (RAC) Chair prior to the scheduled date of the MRMC MS-1 exercise.

The evaluation resulted in no Level 1 (Ll) or Level 2 (L2) Findings.

In Summary: The State and local ORO demonstrated that their plans and procedures could be adequately implemented. The Federal Emergency Management Agency has reasonable assurance that measures can be taken to protect the health and safety of the public in the event of a radiological incident at DCPP.

SUMMARY

2.1 Exercise Purpose and Design The Federal Emergency Management Agency administers the REPP pursuant to the regulations found in Title 44 Code of Federal Regulations (C.F.R.) § 350,351 and 352 (October 2011) and the FEMA REPP Manual (January 2016).

Title 44 C.F.R. § 350 (October 2011) codifies 16 Planning Standards that form the basis for radiological emergency response planning for licensees, State, local, and tribal governments impacted by the EPZ's established for each NPP site in the U.S. of America.

One of the REPP cornerstones established by these regulations is an eight year exercise design cycle of Off-site response capabilities. The EOP Agreement and scenarios for the MRMC MS-1 exercise allow the ORO's the opportunity to implement and demonstrate their plans and procedures to protect the health and safety of the public in the event of a radiological incident occurring at a nuclear plant.

The results of this exercise, together with the review of the RERP's and procedures, the verification of the periodic requirements (found and identified in U.S. Nuclear Regulatory

-Commission Regufation-(NUREGJ-06511-/FEMA:~Radiological -Emergency-Preparedness~

(REP)-1) enable FEMA to provide a statement with transmission of this final After Action Report to the U.S. Nuclear Regulatory Commission (NRC) that State, and local plans and preparedness are:

  • Adequate to protect the health and safety of the public living in the vicinity of the nuclear power facility; and
  • That appropriate protective measures can be implemented Off-site in the event of a radiological incident occurring at the nuclear power facility.

2.2 Exercise Demonstration Criteria, and Core Capabilities The MRMC MS-1 was conducted to provide FEMA the opportunity to evaluate and assess emergency plans and associated implementing procedures, and facilities and equipment, which would be used in a radiological incident occurring at an NPP.

2.2.1 Exercise Demonstration Criteria Off-site Response Organizations demonstrated the following criteria, with references to the 16 Planning Standards, contained in FEMA REPP Manual (January 2016), and associated Core Capabilities from the U.S. DHS National Preparedness Goal (September 2015). The 16 Planning Standards are published in NUREG-0654/FEMA-REP-1, and are identified below as an alphanumeric sub-element.

Assessment Area 1 - Emergency Operations Management Criterion l.a.1: ORO' s use effective procedures to alert, notify, and mobilize emergency personnel and activate facilities in a timely manner. (NUREG-0654/FEMA-REP-1, A.La, e; A.3, 4; C.1, 4, 6; D.4; E.1, 2; H.3, 4)

Criterion l .d. l: At least two communication systems are available, at least one operates properly, and communication links are established and maintained with appropriate

Assessment Area 3 - Protective Action Implementation Criterion 3.a. l: The ORO's issue appropriate dosimetry, KI, and procedures, and manage radiological exposure to EW's in accordance with the plans/procedures. Emergency workers periodically, and at the end of each mission, read their dosimeters and record the readings on the appropriate exposure record or chart. Unclassified Radiological Emergency Preparedness Program After Action Report/Improvement Plan Diablo Canyon Power Plant Assessment Area 6 - Support Operations/Facilities Criterion 6.d.1: The facility/ORO has the appropriate space, adequate resources, and trained personnel to provide transport, monitoring, decontamination, and medical services to contaminated injured individuals. (NUREG-0654/FEMA-REP-1, F.2; H.10; K.5.a, b; L.1, 4) 2.2.2 Core Capabilities The U.S. DHS National Preparedness Goal (September 2015) organizes Core Capabilities into five Mission Areas. Mission Areas serve as an aid in integration and coordination across Core Capabilities to achieve the goal of a secure and resilient nation.

Of the three REPP Assessment Areas and Demonstration Criteria listed in 2.2.1, there are two Core Capabilities which are Common to all the five Mission Areas.

The two Core Capabilities are:

  • Planning; and
  • Operational Coordination.

There are four Core Capabilities which span one of the five Mission Areas: Response.

The four Core Capabilities are:

  • Environmental Response/Health and Safety;
  • Mass Care Services;
  • Operational Communications; and
  • Public Health and Emergency Medical Services.

The six Core Capabilities presented above, are shown below with associated Mission Areas and related REPP demonstration criteria evaluated during the MRMC MS-1:

Common Mission Area Core Capabilities serve to unify the mission areas and, in many ways, are necessary for the success of the remaining core capabilities. Associated diverse activities include:

Common Mission Area Core Capabilities is met by the demonstration of REPP criteria:

Response Mission Area Core Capabilities emphasizes saving and sustaining lives, stabilizing the incident, rapidly meeting basic human needs, establishing a safe and secure environment, and supporting the transition to recovery. Associated diverse activities include:

  • Conduct appropriate measures to ensure the protection of the health and safety of the public and workers, as well as the environment, from all-hazards in support of responder operations and the affected communities.
  • Provide life-sustaining and human services to the affected population, to include hydration, feeding, sheltering, temporary housing, evacuee support, reunification, and distribution of emergency supplies.
  • Ensure a safe and secure environment through law enforcement and related security and protection operations for people and communities located-within- affocted-areas and also for response personnel engaged in lifesaving and life-sustaining operations.
  • Provide lifesaving medical treatment via Emergency Medical Services and related operations and avoid additional disease and injury by providing targeted public health, medical, and behavior health support, and products to all affected populations.

Response Mission Area Core Capabilities is met by the demonstration of REPP criteria:

2.3 Scenario and Extent-of-Play Agreement Summary The August 22, 2018, MRMC MS-1 exercise was based on two scenarios occurring consecutively that invoked emergency response actions to be carried out by the OROs.

A review of the scenarios for the MRMC MS-1 exercise was conducted on June 21, 2018, by the FEMA Region IX RAC Chair who made the final determination that it was adequate to 14

Unclassified Radiological Emergency Preparedness Program After Action Report/Improvement Plan Diablo Canyon Power Plant support demonstration of DHS/FEMA requirements, as well as selected Off-site evaluation criteria per the EOP Agreement, and as provided in the REPP Manual (January 2016).

A summary of the DCPP MS-1 exercise Scenario 1 (Sl) is as follows (times listed are for planning purposes only.):

Initial conditions at DCPP have Unit 1 is at 100% power and operating at normal operating temperature and pressure. Unit 2 is shut down for refueling outage.

Atmospheric conditions have mild temperatures and moderate winds from the North West.

Maximum winds are expect to be less than 12 miles per hour (mph).

At 0800, while performing valve maintenance on reactor coolant system (RCS) 1-455B on the 115' of Containment, a PG&E maintenance worker's right hand slipped while lapping the valve seat. The maintenance workers glove was sliced through which resulted to an injury to their right hand. The Radiation Protection (RP) technician performing job coverage for the maintenance worker instructed them to exit Containment. While exiting Containment, the injured maintenance worker tripped and fell on the stairs sustaining an injury to their left wrist.

An RP technician who witnessed the accident, calls the DCPP emergency line and requests an ambulance. On-site emergency personnel follow emergency protocol and initiate contact with SLO County Medical Communications (MEDCOM).

The injured and contaminated maintenance worker is stabilized and ready for transportation to MRMC via Cambria Community Healthcare District (CCHD) ambulance.

A turnover of the injured and contaminated maintenance worker will occur inside of the Protected Area at the DCPP medical facility: as per BOP Agreement.

Upon successful turnover, DCPP will terminate the On-site portion of the MS-1. The Off-site portion of the exercise will proceed with CCHD ambulance initiating contamination controls and transporting the injured and contaminated maintenance worker to MRMC.

Once CCHD ambulance arrives at MRMC, the injured and contaminated maintenance worker will be transported into the Radiological Emergency Area (REA). Upon successful 15

Unclassified Radiological Emergency Preparedness Program After Action Report/Improvement Plan Diablo Canyon Power Plant decontamination, the injured maintenance worker will be released from the REA, and S 1 will be terminated.

A summary of the DCPP MS-1 Scenario 2 (S2) is as follows (times listed are for planning purposes only):

Atmospheric conditions have mild temperatures and moderate winds from the North West.

Maximum winds are expected to be less than 14 mph.

An Emergency Classification Level (ECL) of Site Area Emergency was declared at 1200 by DCPP. Protective Action Zones (PAZ) 1 and 2 have been evacuated. At 1500 the ECL is upgraded to General Emergency, and at 1600 a radiological release occurs. Cambria Community Healthcare District ambulance and MRMC have been notified of the ECLs and the radiological release occurring at DCPP. Medical Regional Medical Center has been identified to receive and treat injured radiologically contaminated individuals.

At approximately 1800, a kayaker, who was kayaking North of Port San Luis (PSL) comes ashore and prepares to exit from Avila Beach. The kayaker is involved in car accident with another vehicle exiting from the Avila Beach area. The kayaker's vehicle sustains moderate damage. However upon exiting from their vehicle the kayaker complains about chest pain and the other driver calls 911. ~ ---~- --- - -- ----- - ~-- -~ - - - -- -- -~ -- ~-

At the time of the car accident, dose projections at the SLO County EOC predicted that areas in PAZ 1 & 2 could be affected by a release of radiation over the next few hours.

The County of SLO Sheriff's MEDCOM will receive the 911 call and transfer the call to MEDCOM. Upon gathering specifics on the car accident, MEDCOM will identify it as not having occurred within the release area. However, there could be individuals exiting from the affected area.

Cambria Community Healthcare District ambulance will be dispatched, by MEDCOM, to respond to the car accident and follow their radiological procedures. The latter is done as those individuals involved in the car accident might have been the exiting from the area affected by the radiological release.

Unclassified Radiological Emergency Preparedness Program After Action Report/Improvement Plan Diab lo Canyon Power Plant Upon arrival to the vehicle accident, CCHD ambulance staff will initiate appropriate radiological and exposure control measures prior to transporting the injured and potentially contaminated patient to MRMC.

Once CCHD ambulance arrives at MRMC, the injured and potentially contaminated patient will be turned over to MRMC ED staff and transported to the REA. Marian Regional Medical Center Monitoring technicians will frisk the injured patient for potential contamination. The injured patient is found to be free of contamination, released from the REA and transferred to a floor unit for treatment. At that time S2 will be terminated.

Extent-of-Play Agreement Summary:

The purpose of the EOP Agreement is to identify participants, negotiate evaluation areas, demonstration criteria and limitations, identify previous Pis, and any planned deviations from the implementation of the applicable plans that will be evaluated during the MS-1.

Applicable plans included the County of SLO NPP Emergency Response Plans (ERP) and Standard Operating Procedures (SOP), and Pacific Gas and Electric Radiation Control Procedures. The State and OROs will implement applicable elements in these plans and procedures.

The expected outcome, of the OROs, is the demonstration of reasonable assurance that the health and safety of the general public can be protected during an NPP incident.

Pursuant to FEMA policy, federal agencies are not evaluated during an evaluated exercise or drill. Federal actions that result in State or ORO non-compliance of an evaluation area may be documented by the evaluator but will not result in any finding.

The FEMA Lead Evaluator and the County of SLO OES Lead Exercise Controller will terminate the exercise or demonstration when all parties agree that all the required demonstration criteria have been adequately demonstrated and evaluated.

Unclassified Radiological Emergency Preparedness Program After Action Report/Improvement Plan Diablo Canyon Power Plant SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the August 22, 2018, DCPP MS-1 exercise, demonstrating portions of their Off-site emergency response capabilities inside and outside of the EPZ surrounding DCPP.

Each jurisdiction and functional entity was evaluated on its demonstration of selected criteria as indicated in the EOP Agreement and as outlined in the FEMA REPP Manual (January 2016).

3.2 Summary Results of Exercise Evaluation The matrix in Table 3 .1 presents the status of all exercise and demonstration Assessment Areas and sub-element criterion that were demonstrated, for evaluation during the DCPP MS-1 exercise, by all participating jurisdictions and functional entities. The FEMA REPP uses the following alphanumeric key to indicate demonstration status of those criteria evaluated-during-the-exercise and-demonstration:~

Unclassified Radiological Emergency Preparedness Program After Action Report/Improvement Plan Diablo Canyon Power Plant SECTION 4: CONCLUSION The Federal Emergency Management Agency, Region IX, evaluated the MRMC MS-1 exercise on August 22, 2018, outside the EPZ surrounding DCPP. The purpose of the MRMC MS-1 exercise was to assess the level of local emergency preparedness in response to a simulated radiological incident occurring at DCPP. The exercise and demonstration criteria was conducted in accordance with FEMA policies, guidance concerning local RERPs and procedures.

The County of SLO OES submitted the EOP Agreement for the MRMC MS-1 to FEMA Region IX THB on May 24, 2018.

The EOP Agreement was reviewed, determined to be adequate, and approved for the demonstration of all evaluation criteria identified by FEMA Region IX's RAC Chair on May 31,2018.

A copy of the EOP Agreement is maintained by FEMA Region IX NPD / THB, the state of California OES, the County of SLO OES, and is herein incorporated by reference.

Based on the evaluation of the August 22, 2018, MRMC MS-1 exercise the Off-site RERPs for the affected local jurisdictions, site-specific to DCPP, can be implemented and are adequate to provide continued reasonable assurance that appropriate measures can be taken Off-site to protect the health and safety of the public in the event of a radiological emergency occurring at DCPP.

Therefore, Title 44 C.F.R. § 350, approval of the Off-site RERPs and preparedness for the state of California site-specific to DCPP will remain in effect.
