After Action Report/Improvement Plan EXECUTIVE

SUMMARY

On June 27, 2018, a biennial Radiological Emergency Preparedness (REP) Plume Exposure Pathway exercise was conducted around the River Bend Station (RBS), located near St.

Francisville, Louisiana. Personnel from the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA) Region VI, evaluated all activities. Th{, purpose of the exercise was to assess the level of preparedness of State and local responders to react to a simulated radiological emergency at RBS. It was held in accordance with FEMA's policies and guidance concerning the implementation of state and local REP plans and procedures.

The previous exercise at this site was conducted on June 29, 2016. The first qualifying exercise to satisfy FEMA rule 44 Code of Federal Regulations (CFR) 350 requirements for Nuclear Regulatory Commission (NRC) licensing to operate the facility was conducted in January 1985.

Including the exercise on June 27, 2018, there have been nineteen DHS/FEMA Region VI evaluated exercises plus several drills conducted since then.

FEMA Region VI wishes to acknowledge the efforts of the many individuals in the State of Louisiana, West Feliciana, East Feliciana, Pointe Coupee, East Baton Rouge and West Baton Rouge Parishes who participated in this exercise. Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. ~till others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities. Cooperation and teamwork of all the participants was evident during this exercise..

This report includes the identification of any exercise findings and recommendations for corrective action where appropriate. All state and local organizations, except where noted in this report, demonstrated an adequate knowledge of the emergency response plans and procedures '

and properly implemented them. There were no Le\\7el 1 Findings, two Level 2 Findings that were corrected during the exercise, and one Plan Issue identified during the exercise. Also, one previous Level 2 Finding was resolved during the exercise.

After Action Report/Improvement Plan SECTION 2: EXERCISE DESIGN

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region VI Office evaluated the exercise on June 27, 2018 to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans and Procedures to protect the public health and safety during a plume emergency involving River Bend Station (RBS). The purpose of this report is to represent the results and findings on the performance of the off site response organizations during a simulated plume emergency.

2.2 Exercise Objectives, Capabilities and Activities Exercise objectives and *identified Capabilities/REP Criteria selected to be exercised are discussed in the Exercise Plan (EXPLAN), Appendix E.

2.3 Scenario Summary The exercise scenario was developed to evaluate the response of exercise participants to an incident requiring evacuation of the public from the 10-mile Emergency Planning Zone surrounding the River Bend Station (RBS). The exercise scenario provided for the evaluation of the State of Louisiana, East and West Baton Rouge, East and West Feliciana, and Pointe Coupee Parishes to make protective action decisions for the public and emergency workers.

After Action Report/Improvement Plan SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the June 27, 2018 exercise to test the offsite emergency response capabilities of state.and local governments in the 10-mile Emergency Planning Zone surrounding the River Bend Station.

Each jurisdiction and functional entity was evaluated on the basis of its demonstration of criteria delineated in the exercise evaluation areas as outlined in the January 2016, Radiological Emergency Preparedness Program Manual. Detailed infonnation on the exercise evaluation area criteria and the extent of play agreement used in this exercise are found in Appendix E of this report.

3.2 Summary Results of Exercise Evaluation The matrix presented on the following page presents the status of all exercise evaluation area criteria that were scheduled for demonstration during this exercise by all participating jurisdictions and functional entities. Exercise criteria are listed by number and the demonstration status is indicated by the use of the following letters:

After Action Report/Improvement Plan POSSIBLE CAUSE: The exact reason that the time the broadcast of the EAS message was not coordinated with the Parishes, is unknown. It may have been caused by *a rush to complete the ANS sequence, a lack of training, or a lack of knowledge regarding the LPRRP Basic Plan.

With all the Parishes on the conference call, and with a coordinated time to sound the sirens already completed, communicating/coordinating the time for broadcast of the EAS message would have been the next logical step before ending the call.

None of the Parishes inquired about, or confirmed, the EAS message time before the call concluded, nor did GOHSEP communicate the EAS message broadcast time before concluding the call..

REFERENCE:

NUREG-0654 E.5 and E.7 LPRRP: Basic Plan-Chapter 4, Paragraph ILE, which states: "An internet-based Emergency Alert System (EAS) program is operated by GOHSEP for River Bend Station. The system has the capability to send prerecorded and original messages to radio stations or commercial television stations as both voice and text. The system can call homes within a sp7cific area, send TTY and SMS messages and broadcast in other languages. Messages and broadcast times are coordinated with the Parish."

EFFECT: The Parishes assumed the EAS message was going to be broadcast at 1253, right after the 3 minute duration of the siren sounding, which commenced at 1250. Instead GOHSEP set the time for the broadcast of the EAS message to be 10 minutes from the start of the sirens sounding, at 1300. This resulted in the Parishes making false assumptions regarding the EAS broadcast time.

The unusually long 10 minute gap between the start of the sirens sounding and the broadcast of the EAS message may also have resulted in some residents in the affected areas to tune in too late or too early to their local EAS station for critical information, such as the evacuation being ordered.

RECOMMENDATION: Modify the appropriate plans/procedures to better define the entire ANS coordination process/sequence.

Add the coordination of the EAS broadcast time step to the existing EMERGENCY PLANNING INFORMATION AID that GOHSEP used during the exercise.

After Action Report/Improvement Plan CONDITION: Kl recommendation for Emergency Workers:

1246 fax was received by Governor's Office of Homeland Security and Emergency Preparedness (GOSHEP) authorizing the administering of Potassium Iodide (KI) to all emergency workers.

At 1320, after-more inquiries for verification, the EOC Director called the GO SHEP to determine if KI had been authorized. During the call the EOC Director noticed fax that was sent at 1246. During brief from EOC Director it was discovered the fax did not list East Feliciana Parish (EFP) for KI authorization. The EOC Director determined since the fax did riot include the EFP and the time had been so long since the recommendation, that it didn't have cause for any further effort. The recommendation did include any emergency workers within a 5 mile radius of the utility. However, the emergency workers and public. were not impacted due to the scenario.

After inquiry from the evaluator the EOC Director called the GOSHEP again to verify that EFP should not be on the recommendation. At 1338, a fax was received from the GOSHEP with the KI recommendation for emergency workers including EFP. 1340 the EOC Director briefed that KI recommendation for ingestion be communicated to emergency workers.

Emergency Classification Levels:

0829 River Bend Station Short Notification Form was received that an Alert had been declared 0948 River Bend Station Short Notification Form was received that a Site Area Emergency had been declared. After the SAE notification, per procedures notifications were made and call down checklists were performed. Preplanning of PAR's were performed for schools and special needs.

1000 River Bend Station Notification Message Form was received that the Emergency Classification Level was at an Alert. The decision from the Parish President, EOC Director and Sheriff was that the declaration had been down-graded to and Alert. At that time the EOC staff conducted themselves as if the ECL was at an Alert and not at an SAE. At no time did the decision making entities ever verify the down-graded ECL. With no verification of the ECL from the EOC Director, the lead evaluator stopped play to communicate to the leadership group that they needed to verify what ECL they were making decisions in or it would impact the exercise play. At that time the EOC Director contacted the GOSHEP and it was verified the correct ECL was indeed an SAE. Decision process was started again to reflect the correct and current ECL.

After Action Report/Improvement Plan APPENDIX E: EXERCISE PLAN SCENARIO NARRATIVE The scenario has been prepared to test the effectiveness of the emergency response organization (ERO) and functions of the RBS Emergency Plan, implementing procedures, facilities, and equipment. Some events that occur during this drill are unrealistic and may not be strictly in accordance with the design basis of the Facility.

Summary At 0805 Receive report from the Fuel Building (DFS Project Manager) that the Spent Fuel Cask Crane has malfunctioned. The crane and its suspended load (loaded Hi-TRAC container) have stopped inside the Fuel Building.

At 0815 the Radwaste NEO performs actions to discharge Radwaste Recovery Sample Tank TK4A. A human error has occurred where the discharge permit is written for the wrong radwaste tank. Radwaste Tank B has been sampled, but Tank A is being discharged. After the discharge begins, RMS-RE-107 alarms with a reading> 1.27E-:01 µCi/ml. Equipment operators are unable to secure the leak within 15 minutes. The Emergency Director will declare an Alert on AAl-2, RMS-RE107effluent monitor has a VALID reading> 1.27E-01 µCi/mlfor > 15 minutes. Offsite notifications will be made and the ERO will be augmented using the ERO callout system. The Technical Support Center (TSC), Operations Support Center (OSC), Emergency Operations Facility (EOF) and Joint Information Center (JIC) become operational.

A catastrophic jet pump failure occurs causing reactor power to lower. Mechanical clad damage begins to occur due to jet pump parts entering the core. Off-gas system pre-treatment radiation levels will rise. The crew will recognize that fuel damage has occurred and determine that it is necessary to shutdown the unit. A manual reactor scram will be initiated however, all rods will not insert and reactor power will remain above 5%. High pressure feed will be available with condensate and feedwater. Both reactor core isolation cooling (RCIC) and high pressure core spray"(HPCS) are available. Pressure control will be with safety relief valves (SR Vs) and main steam isolation valves (MSIVs). The Emergency Director will consult the EALs and at 1115 declare a Site Area Emergency based on SS3-1, an automatic'scram failed to shutdown the reactor AND Manual actions taken at the reactor control console do not shutdown the reactor as indicated by reactor power ~ 5%. Offsite notifications will be made and the protected area evacuated of nonessential personnel. A leak in the reactor water clean-up (RWCU) system will 32 I

The running control rod drive (CRD) pump will trip offline. The second CRD pump will be started to allow operators to manually insert control rods. The leak in RWCU will propagate resulting in a release through the turbine building and main plant exhaust. Radiation levels increase in the turbine building. Reactor water level will slowly lower, but will still be above the top of active fuel (-162").

At approximately 1235, the Emergency Director will consult the EALs and declare a General Emergency based on AG 1-2, Dose assessment using actual meteorology indicates doses > I OOOmR TEDE or 5000 mR thyroid CDE at or beyond the site boundary.

Note: If the operations crew cannot maintain RPV pressure and Suppression Pool temperature in the Heat Capacity Temperature Limit (HCTL) Safe Zone, then the General Emergency will be declared on SG3-1. The classification and protective action recommendation will be communicated to offsite agencies. The release will be a monitored release.

Main plant exhaust rate will reach its highest level at approximately 1250. The TSC and OSC teams will successfully isolate the RWCU leak resulting in a decline in radiological release rate.

Radiological release rates will fall to near background levels around 1330. At approximately 1400, the simulator will be placed in FREEZE and the players will begin a self-critique.
