ATTACHMENT 1 Re: Turkey Point Unit 4 Personnel Exposure A Westinghouse employee reported to the Turkey Point Plant on December 4, 1976 for the purposes of performing Eddy Current, testing and tube plugging on the .Unit 4 steam generators. The employee reported to the site with no previous occupational exposure for the fourth quarter of 1976. Measured dose r'ate in the primary side of the steam generator was approximately 15 Rem/hour.

As the employee's exposure approached the company's quarterly guideline values, his TLD was processed on December 6, 1976.

The results were below guideline values, and the employee was issued a new TLD and permitted to return to work. The employee finished his work and left the Turkey Point Plant site on the evening of December 7, 1976. The results of the first TLD reading added to subsequent pocket-dosimeter results indicated that no applicable limits had been exceeded.. However, when the second TLD was processed, the employee's total exposure for the quarter exceeded the 3 Rems allowed by 10 CFR 20.101(b)(1).

The cause of the exposure was a lower reading on the pocket-dosimeter than was recorded from the TLD.

As a means to preclude similar occurrences, each person entering the steam generator channel heads will be issued two self-reading pocket, ionization chambers (dosimeters) and the one indicating the highest exposure will be used in computing the individual's radiation exposure.

On result was 1810 mrem. On December 7, 1976, Mr. Paris received 815 mrem by pocket dosimeter. The TLD results of 1810 mrem and the pocket dosimeter results of 815 mrem gave Mr. Paris an accumulated whole body exposure of 2625 mrem for the quarter.

Mr. Paris left the Turkey Point Plant site on December 7, 1976.

On December 15, 1976, Mr. Paris'econd TLD reading was obtained and the result was 1210 mrem.

I Summing the two TLD results for exposure received at the Turkey Point Plant gave an exposure of 3020 mrem.
