F ABNORMAL OCCURRENCE NO. 251-75-9 TURKEY POINT UNIT NO.
4 DATE OF OCCURRENCE:
JUNE 13, 1975 BREACH OF CONTAINMENT INTEGRITY AIRLOCK EQUALIZING VALVE OPEN A.
CONDITIONS PRIOR TO OCCURRENCE Unit No.
4 was shutdown and Reactor Coolant System temp-erature was about 350oF.
A heatup was in progress using reactor coolant pumps.
The unit had been shutdown for refueling since March 29 and was being returned to ser-vice.
B.
DESCRIPTION OF OCCURRENCE At about, 3:45 P.M. on June 13, 1975 a Quality Control inspector discovered a disconnected mechanical linkage on the equalizing valve for the outer door of the per-sonnel access airlock.
Further investigation showed that the valve was open thereby establishing a flow path from containment to atmosphere whenever the airlock inner door was open.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE Containment integrity had been administratively verified prior to plant heatup by completion of the Prestart Check-Off List (Turkey Point Units No.
3 and 4 Operating Pro-cedure P2Pl.l).
However, personnel conducting the check-off were not aware the linkage was disconnected because both valve and linkage are hidden from view behind a
vertical steel cover plate.

- Rusche, Director The valve operating handle and position indicators are easily visible in front of the steel plate.
Thus, per-sonnel thought they were checking the valve shut when in fact it was staying open.
The valve had, been positively,,demonstrated to be shut on March 20,,1975 by successful completion of,an'irlock leak test.
Apparently the valve linkage -was disconnected and the valve left in the open position sometime after that
- date, probably during the refueling shutdown period.
The reason for this occurrence has not been determined.
D.
ANALYSIS OF OCCURRENCE Breach of ~c'ontainment integrity occurred for brief periods whenever the inner airlock door was opened concurrent with movement of fuel inside containment or heatup of the Reactor Coolant System above 200oF.
The flow path was through a two inch equalizing valve which permitted only a small amount of air flow from containment to atmosphere.
However,'his occurrence did not prevent. the establishment of containment integrity because the airlock inner door and its equalizing valve were operable.
Therefore, the health and safety of the public were not adversely affected.
E.
CORRECTIVE ACTION The personnel access airlock inner door and its equalizing valve were checked shut to confirm containment integrity.
The equalizing valve linkage was connected and the valve was shut at about 5:00 P.M. on June 13, 1975.
id A check point shall be added to the "blind flange list" to require verification that the mechanical linkages for the airlock inner and outer door equalizing valves are connected.
Satisfactory completion of this list is re-quired by Operating Procedure p2pl.l, Prestart Check-Off List, as a part of verifying containment integrity.

FAILURE DATA Breach of containment integrity occurred on one other occassion.
It was reported as Abnormal Occurrence 251-75-6.
