& Light Company and Narendra K. Bhatia, Bechtel Corporation, describing the sequence of events and causitive mechanism involved in the failure of the safety valve headers at Turkey Point Unit No.
3 on December 2,
- 1971, and the post-incident investigation conducted by Florida Power
& Light Company, Bechtel Corpora-tion and Battelle Memorial Institute.
The testimony also describes the replacement of the failed safety valve headers and the analyses associated with the replaced components.
Attached to this testimony is the safety valve header Inci-dent Report submitted to the Atomic Energy Commission, re-vised as of March 8, 1972.
Witnesses Olmstead and Bhatia will sponsor the introduction of this report.

Testimony of Mr. Gerard W. Gartland of Bechtel Corporation describing the studies involving a complete re-evaluation of all stresses on the safety valve headers in Turkey Point Units Nos.
3 and 4 as initially designed and as subsequently modified following the above-referenced incident.

Testimony of Mr. R.
E. Monroe of Battelle Memorial Institute describing metallurgical and other studies conducted by Mr.
Monroe and his associates which conclude that there is no evidence of material or fabrication deficiencies in the safety valve headers, but that the failure occurred as the result of a sudden overload being applied to the piping system from the opening of one or more of the safety relief valves.
Mr.
Monroe's testimony also describes his evaluation and conclusions with respect to the adequacy of the design of the replacement safety valve headers.

Testimony of Mr. Charles P. Currey of Westinghouse Electric Corporation describing an analysis of the transient associated with the failure of the safety valve headers conducted by Westinghouse.
The analysis of reactor coolant system components determined that the stresses, the pressure and the temperature changes associated with the transient, were within the design capability of the components involved.
Mr. Currey's testimony also describes the inspection of components potentially.affected by the incident and the results of the inspection.
Attached to the testimony described in l. above is a simplified diagram of the safety valve header arrangement at Turkey Point Unit No.
3 which Applicant hopes will be of assistance to the ASLB in its consideration of this matter.
Applicant will have at the prehearing conference a scale model of the safety valve header arrangement which may be of further assistance to the Board.
In addition to the foregoing, Applicant will submit as early as possible prior to the prehearing the results of a test program to be conducted at Turkey Point Unit No.
3 for a period of about ten (10) days duration beginning about April 26, 1972.
The test program will duplicate the conditions of pressure and temperature experienced during the pre-operational test of Unit No.
3 on December 2,
1971 when the safety valve header incident occurred.

- 1972, the Notice of Hearing in this proceeding (p.2) and the Board's Order of April 10, 1972. It is Applicant's expectation that the foregoing will establish (1) the sequence of events involved in the specific component failure (2) the causative mechanism associated with the failure (3) the adequacy of the post-incident investigation and the sufficiency of the corrective action taken by the Applicant.
Applicant would be pleased to provide such additional information as the Board may desire involving the matters described herein,
- and, if possible, in the time remaining prior to the prehearing conference, Applicant will be grateful to receive any questions the Board may have with respect to this material.
Finally, enclosed is the statement of James
- Coughlin, a Vice President of Applicant, in the nature of opening remarks, which has been prepared in conformity with 10 CFR Part 2, Appendix A, Section III, (c) (1).
Applicant requests that the testimony described in Items 1-4, above be incorporated in the record as though read and that the wit-nesses sponsoring such testimony be examined as necessary on a "round table basis" in accordance with 10 CFR Part 2, Appendix A, Section III (c)
(4) and (6).
