Dear Mr. Keppler:

This is to confirm our telephone call to Mr. K.R. Baker, NRC Region III, at 1432 hours0.0166 days <br />0.398 hours <br />0.00237 weeks <br />5.44876e-4 months <br /> on October 29, 1976. During that conversation we reported that an abnormal environmental occurrence had taken place October 29, 1976.

This occurrence was the failure to monitor the circulating water total chlorine residual during the chlorination cycle in violation of Appendix "B" Technical Specifications Section 2.2. 1.3.

The chlorination program is presently set to chlorinate automatically at 1000 and 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br /> each day and; due to the Chemical Technician being unaware of the time, no monitoring=was performed at the 1000 hour0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> period.

We are continuing our investigation and will submit the required report within seven days as requi red by Appendix "B" Technical Specifications Section 5.4.2.1.

Based on our review of the information contained in your letters, we have d termined that individuals licensed on Unit 1 who successfully complete the Differences lraining Program v,ill b>> eligible to receive licenses for Unit 2 without liRC administered e.,amination. Upon meeting the other re-quirements of Section 55.11, their licenses :>ill be amended, at the appropriate time, to include operation of Unit 2.

Dear Mr. Keppler:

t<ith reference to my letter to you of April 28, 1977, in which I reported the possible loss of an unirradiated non-radioactive incore movable detector, I am pleased to report that the detector in its wooden shipping container has been located. It was in the controlled area of the auxiliary building.

To prevent recurrence of this type of incident,. our Special Nuclear Material

'-Accountability Procedure; PMP 4040 SNH.001, has.been revised to give the

  • Nuclear Materials Manager direct responsibility for preparation of the necessary shipping documents and ensuring the detector is shipped. Also, in addition to the normal shipping documents, an NRC/ERDA-741 form shall be completed by the Nuclear Materials Manager and mailed to the company receiving the detector the same day the detector leaves the plant site. By use of the 741 form for shipment, we will be informed by the receiving company when the de te ctor a rri ves. at th ei r fa ci 1 i ty.

Rather than administer a second examination to these men, you requested that their "final examinations" pnior to licensinq be used for determining mandatory lecture attendance pursuant to the requirements of your requalification program.'ased on our review of the information contained in your letter, we have determined that the operator requalification examination may be waived and that the "final examinations" may be used as a basis for determining mandatory lecture attendance. The individuals involved are Messrs. Boone, Freehlinq, Cooledqe, Gallagher, Hall, Runser, Skinner, Ziomerman and Schwerha.

The applications for Hessrs. )randolph, Tollas, Strasser, Craig, Foss, Lewis, Vonk and Perrine do not contain the certifications from the Combustion Engineering Training School. Prior to the administration of the operating tests, these certifications must be forwarded to this office. Also, Hr. Skinner s application indicated that his Certificate of'edical History has been fo~~carded to this office. To date, we have not received the Certificate.

The examinations have been assigned to Mr. A. .. Vinther and fir. T. E.

Edwards. The examinations are schedu'led for the week of January 17, 1977.
