ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric and Power Company Surry Docket No. 50-280 License No. DPR-32 During an NRC inspection conducted on March 23-27 and April 6-10, 1992, a violation of NRC requirements was identified. In accordance with *the."General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1990), the violation is listed below:

Surry Technicat Specifications, Section 4.4, Item 2.2, and associated Table 4.2-1, as implemented by site Augmented Inspection Manual, requires that the Recirculation Spray Rings be visually and liquid penetrant inspected. The liquid penetrant inspection is to include six patches (each nine inches square) evenly distributed around the ring. The visual inspection is to cover the full length of pipe.

10 CFR 50, Appendix B, Criterion V, requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures........ and shall be accomplished in accordance with these instructions, procedures...........

Paragraph 1.5.3 of licensee procedure NDE-PT-201, the applicable procedure for liquid penetrant inspection, requires that the surface to be examined and adjacent areas within at least one inch shall be dry, and free of all dirt, grease,........... and other extraneous matter that couid obscure surface openings or otherwise interfere with the examination.

Contrary to the above, on March 24, 1992, activities affecting quality were not being accomplished in accordance with instructions and procedures in that, for one Recjrculation Spray Ring (ISI Items AE-063, 064; 065, 068, 069 and 070), three of the nine inch patches were liquid penetrant inspected without removing blotches of paint and other foreign materials (marking materials). In addition only a nine inch band around the header at each

. liquid penetrant patch, in lieu of the full length of pipe, was being visually inspected.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company is hereby required to submit a written statement or explanation: to the U.S. Nuclear Regulatory

  • Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region Il, and a copy to the NRC Resident Inspector Surry, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).* This reply should be clearly marked as a "Reply to a Notice of Violation" and should included [for each violation]: (1) the reason for the violation, or, if_contested, the basis for disputing the violation, (2) the corrective steps that will be taken and the results achieved; (3) the 9205270151 6;88b~eo PDR ADOCK.
