(INSPECTION REPORT NOS. 50-280/92-07 AND 50-281/92-07)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
Mr. M. W. Branch of this office on March 8, through April 4, 1992. * The
inspection included a review of acti vi ti es authorized for your Surry faci 1 i ty.
At the conclusion of the inspection, the findings were discussed with those
members of your staff identified in the enclosed inspection report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progres~.
Based on th~ results 6f this inspection, certain activities appeared to be in
violation of NRC requirements, as specified in the enclosed Notice of Violation
(Notice).
The enclosed Notice contains two separate violations.
One violation
involved breaches of containment integrity during refueling. This violation is
of concern because your procedures did not adequately address the control of
maintenance activities on systems_ that were required to be closed for contain-
ment integrity.
The second violation involved the failure of maintenance
personnel to prevent foreign material from entering the service water system.
This violation is of concern because it limited the containment recirculation
spray systems ability to operate as designed.
You are required to respond to th~s letter and should fol1ow the instructions
specified in the enclosed Notice when prepa-ring your response. - In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice", a copy of
this letter and its enclosures will be placed in the NRC Public Document Room.
